I hereby certify that I am the adult parent or guardian of First Name Last Name , a minor under the age of eighteen years, and I consent to their participation in recreational activities (skateboarding) and use of the skateboards, safety equipment and ramps, operated and maintained by Shred Education. I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of property damage, personal injury or fatality) of (1) said minor child’s participation in recreational activities and (2) their use of the skateboards, safety equipment and ramps. I represent and warrant that the child named below is in good health and does not suffer from any medical condition that would prevent their voluntary participation and enrolment in any of the above activities. I further represent and warrant that it is my responsibility to inform the club should my child’s medical condition change to the extent that further participation in any of the aforementioned activities be detrimental to their health and wellbeing. I further understand and agree that Shred Education supplies the skateboards, safety equipment and ramps “as is”, and that Shred Education disclaims all warranties, expressed or implied, including warranties of merchantability and fitness for a particular purpose. I acknowledge and agree that my child participates in said activities entirely at their own risk and consequentially waive and relinquish all claims they may have for property damage, personal injury or fatality suffered by my child as a result of their participation, and I hereby release Shred Education or any club official or any club member from any liability for such property damage, personal injury or fatality to the fullest extent permitted by English Law.
