1v1 Ballerz Tournament Registration Form

Format of play

-16 Ballerz will participate in a 1v1 Tournament to crown the top 1v1 Baller.

-Round of 16 formats, each round players will get 3 attempts. Point is awarded for a goal scored or defensive stop, GK saves do not count.

-2 Goalies (Will rotate for each duel)

-Winner will receive a prize, and the runner up will receive a consolation award.

What will they gain?

A platform to showcase their 1v1 abilities, skill, drive and determination to win.

What we provide?
A positive structured setting to express their ability and have fun. It will be competitive, and players will be encouraged to put maximum effort and desire into play.

When will it occur?

Organized once a month for Competitive Players.

RULES

-Each player has 3 attempts.

-Point is awarded for a goal scored or a defensive stop is made (e.g. tackle of the ball or regain of possession). Attacking player missing a shot or kicking the ball out does not count, nor does a GK save count.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all soccer activities prepared by the ES Soccer Training during the selected session. In exchange for the acceptance of said child’s candidacy by the ES Soccer Training Free Play Day, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless ES Soccer Training and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected session.

In case of injury to said child, I hereby waive all claims against ES Soccer Training, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death. I agree to allow the use of photos obtained by ES Soccer Training throughout the session program. I have carefully read the information above and fully understand its contents and voluntarily agree to its terms. *
