    You must call the Youth Ministry Office to make an  appointment to fill out a form.

Peter and Paul Catholic Church, and the Archdiocese of San Antonio from any liability for injuries (including death) sustained by my child in connection with their participation in these programs and activities while under the supervision of the sponsors of these programs.*
