PARENT PERMISSION AND WAIVER FORM
I/We, the parents/guardians of the student named above, understand the nature of the classes and activities that take place at: 4676 Barranca Pkwy Second floor of, Center, Irvine, CA 92604.
We hereby grant permission for our son/daughter to participate. We understand that adequate and appropriate supervision will be provided. In the event of an injury requiring medical attention, I hereby grant permission to the supervising teacher(s) or staff (including volunteers) to attend to my son/daughter. If the injury warrants further medical attention, I expect every effort will be made to contact me to receive my specific authorization before action is taken. If efforts to contact me are unsuccessful, I grant permission for necessary medical treatment to be given. In addition, I hereby give my permission to the supervising teacher(s) or staff (including volunteers) to take my child to the physician, dentist, or to the hospital if an accident or serious illness occurs on the trip and I cannot be located.
We recognize, however, that unanticipated situations and problems can arise on any trip, school-sponsored or otherwise, which situations or problems are not reasonably within the control of the supervising teacher(s) or staff (including volunteers). We further agree to release and hold harmless the directors, agents, officers, employees, and Magikid Robotics Lab in Irvine from any and all liability, claims, suits, demands, judgments, costs, interest and expense, (including attorneys’ fees and costs) arising from such activities, including any accident or injury to the student and the costs of medical services, or any cause beyond the control of Magikid Robotics Lab in Irvine, including, but not limited to, natural disasters, civil disturbances, acts of terrorism, and wars.
Magikid Robotics Lab Irvine reserves the rights to expel, suspend, or refuse to provide service to anyone due to any situation we see fit. In the event of expelling and suspension, only unused tuition is qualified for refund or credit, used tuition is non-refundable and non-transferable.
PARENTS & STUDENT SAFETY
Please make sure your student understand that coming to Magikid Robotics Irvine is a fun privilege. Our goal is for every student to come in and leave happily. Your student’s safety is extremely important to us. To make sure everyone has a fun and safe time at the lab, we need to make sure everyone knows the rules. These include no running or leaving the classroom without the instructor’s permission (including bathroom breaks), following instructions on how to properly use materials, and always listening to the teacher. We have a strict zero-tolerance policy on bullying. If we see bullying, fighting, or violent behavior including throwing and pushing items or people your child will be asked to leave.
Additionally, please let us know if you plan on having someone other than you pick up your child. We will make sure that your child is picked up by someone both you and they trust.
Absolutely no smoking, drugs, or alcohol are permitted on the premises, whether indoors or outside.
PHOTO AND VIDEO RELEASE
By signing up for services you agree to the Magikid Robotics Lab Photo and Video Release policy. You may request to see the photos or videos taken, and/or opt-out at any time. The policy includes the right to take still photographs and motion pictures of students and families during lab-related activities, and use them for marketing purposes without compensation, with protection from legal action.
Social media platforms include, but not excluded to, Facebook and Instagram at the time of this current edit.
BEHAVIOR ISSUES AND EXPECTATIONS OF STUDENTS
Instructors will notify managers and parents of student misbehavior. Students with repeated or extreme behavior issues may be asked to leave the class. Our hope is that our students come in ready to have a fun and educational experience at our lab. If after several warnings the student in question is unable to cooperate with lab policy by making the learning environment unsafe or difficult for others, they may be asked to leave indefinitely. We ask for the parents’ help in making sure their student is ready for each class.
Refund Policy
There will be no refund provided after the payment was done.
We reserve the right to refuse service to anyone for any reason within the expectations of federal anti-discriminatory laws.
