Liability Waiver — I understand that participation in Hoops on King programs involves physical activity and inherent risk of injury. I hereby release and hold harmless Hoops on King, its coaches, staff, and affiliates from any and all liability for injury, illness, or loss arising from my child's participation.
Liability & Agreements
Medical Authorization — I authorize Hoops on King staff to seek emergency medical treatment for my child if I cannot be reached. I understand I am responsible for all associated medical costs.
Media Release — I grant Hoops on King permission to photograph and/or video record my child during sessions for use on social media, marketing materials, and the Hoops on King website.
Code of Conduct Agreement — I have read and agree to the Hoops on King Code of Conduct. I understand that my child may be dismissed from the program without refund for conduct that violates program standards.
Photo ID Pickup Policy — I understand that my child will only be released to adults listed on this registration. A valid photo ID may be required at pickup.
Refund Policy — I understand that all registration fees are non-refundable. In the event of a program cancellation by Hoops on King, a full refund or credit will be issued based on time spent.
