The Summit Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard; we continually strive to reduce risks and insist that all participants follow safety rules and instructions designed to protect everyone’s wellbeing. However, participants and parents/guardians of minors must recognize that there is an inherent risk of injury when choosing to participate in recreational activities and programs, and you are solely responsible for determining if you or your minor child/ward are physically fit and appropriately skilled to participate; it is strongly recommended that you consult a physician prior to participation, especially if the participant has experienced illness, injury, disability, or impairment. Recreational programs and activities are intended to challenge and engage participants physically, mentally, and emotionally, and despite careful preparation, instruction, medical advice, conditioning, and the use of proper equipment, there remains a risk of serious injury, including but not limited to slipping, tripping, or falling; collisions or contact with people or objects; weather-related hazards; improper skill level or conditioning; carelessness or horseplay; premises or equipment defects; and inadequate supervision, instruction, or officiating; it is impossible for the Summit Park District to fully eliminate all risks, and participants must acknowledge and accept these inherent risks. By registering for and participating in this program/activity, you understand that you are assuming all risks associated with participation, including those related to transportation services when provided, and you acknowledge that you and/or your minor child/ward may sustain injuries or damages arising out of participation; you voluntarily assume full responsibility for any and all injuries, damages, or losses, regardless of severity, that may be sustained. On behalf of yourself and/or your minor child/ward, you agree to waive, release, and relinquish all claims against the Summit Park District, its officers, officials, agents, employees, and volunteers arising from participation in this program/activity, and you do hereby fully release and forever discharge the Summit Park District from any and all claims for injuries, damages, or losses that may accrue in connection with participation, to the fullest extent permitted by Illinois law. You further acknowledge that all participants are expected to demonstrate respectful and appropriate behavior and agree that the Summit Park District reserves the right to remove any participant whose behavior is disruptive, unsafe, or inappropriate without refund; you acknowledge that you have reviewed and understand the District’s refund and cancellation policies, including applicable timelines, non-refundable fees, and the District’s right to cancel or modify programs due to low enrollment or unforeseen circumstances; and you agree to abide by the Summit Park District Code of Conduct, which promotes a safe, inclusive, and respectful environment, understanding that harassment, discrimination, bullying, or unsafe behavior may result in removal from programs and/or suspension from future participation. You grant permission to the Summit Park District to photograph and/or record you or your minor child/ward for promotional purposes, including use in brochures, social media, websites, and other publications, without compensation. By acknowledging this agreement, you certify that you have read, understand, and agree to all terms stated above and that you are at least 18 years of age or the legal guardian of the participant.
