A friendly reminder that even if your child is does not have allergies, MetaMinds Stem Academy is a nut free facility, and we ask that parents do not pack their children with any nuts, including peanut butter, or any other common allergens, for the safety of all attendees. Thank you!

A refund for MetaMinds STEM Academy’s 2026 Summer Camp may be issued back to your original method of payment within three (3) days of submitting your registration. Please note that refunds will not include any transaction fees charged by credit card processors (3%).&#13;&#10;&#13;&#10;After the three-day refund window has passed, any refund requests made more than one (1) calendar month prior to your selected camp start date will be issued as “Class Credit.” &#13;&#10;&#13;&#10;Class Credit is valid for one (1) year from the original booking date and may be applied toward any service, class, or program offered at MetaMinds STEM Academy.

PARENTAL CONSENT, ASSUMPTION OF RISK & LIABILITY WAIVER&#13;&#10;I/We, the undersigned parent(s) or legal guardian(s) of the student named above , hereby consent to the Student’s participation in all classes, camps, workshops, programs, and related activities conducted here at MetaMinds STEM Academy.&#13;&#10;&#13;&#10;ASSUMPTION OF RISK&#13;&#10;&#13;&#10;I/We acknowledge that participation in STEM activities, robotics, technology use, classroom instruction, movement within the facility, and related educational programs involves inherent risks, including but not limited to property damage, illness, personal injury, or death.&#13;&#10;&#13;&#10;I/We voluntarily and knowingly assume all risks, both known and unknown, foreseeable and unforeseeable, associated with the Student’s participation.&#13;&#10;&#13;&#10;MEDICAL AUTHORIZATION&#13;&#10;&#13;&#10;I/We authorize the Academy’s owners, directors, managers, instructors, employees, agents, and volunteers to provide basic first aid and to secure emergency medical treatment if deemed necessary.&#13;&#10;&#13;&#10;Reasonable efforts will be made to contact the parent/guardian prior to treatment. If contact cannot be made, I/We authorize medical care and transportation to a hospital, urgent care facility, physician, or dentist as deemed necessary.&#13;&#10;&#13;&#10;I/We accept full financial responsibility for any medical expenses incurred.&#13;&#10;&#13;&#10;RELEASE AND WAIVER OF LIABILITY&#13;&#10;&#13;&#10;To the fullest extent permitted by Texas law, I/We hereby release, waive, discharge, and hold harmless MetaMinds STEM Academy, its owners, directors, officers, employees, agents, contractors, instructors, volunteers, and representatives from any and all claims, liabilities, demands, actions, damages, costs, or expenses (including attorneys’ fees) arising out of or related to:&#13;&#10;&#13;&#10;Participation in Academy programs&#13;&#10;&#13;&#10;Use of equipment or facilities&#13;&#10;&#13;&#10;Injury, illness, accident, or property damage&#13;&#10;&#13;&#10;Acts or omissions of other participants&#13;&#10;&#13;&#10;Circumstances beyond the Academy’s control&#13;&#10;&#13;&#10;This release includes claims arising from ordinary negligence, except in cases of gross negligence or willful misconduct as prohibited by law.&#13;&#10;&#13;&#10;FORCE MAJEURE&#13;&#10;&#13;&#10;The Academy shall not be liable for cancellations, disruptions, closures, or schedule changes resulting from events beyond its reasonable control, including but not limited to natural disasters, pandemics, public health emergencies, government orders, civil unrest, war, terrorism, or utility failures.&#13;&#10;&#13;&#10;No refunds or credits shall be issued for such events unless determined at the Academy’s sole discretion.&#13;&#10;&#13;&#10;STUDENT CONDUCT & DISCIPLINE&#13;&#10;&#13;&#10;Students must follow all Academy rules and staff instructions.&#13;&#10;&#13;&#10;The Academy maintains a strict zero-tolerance policy for bullying, harassment, fighting, violence, or unsafe conduct.&#13;&#10;&#13;&#10;The Academy reserves the right, at its sole discretion, to suspend, dismiss, or refuse service to any student whose behavior is disruptive, unsafe, or inappropriate.&#13;&#10;&#13;&#10;In cases of dismissal, used tuition is non-refundable and non-transferable. Only unused tuition, if any, may be eligible for credit at the Academy’s discretion.&#13;&#10;&#13;&#10;PROPERTY DAMAGE&#13;&#10;&#13;&#10;Students and parents/guardians agree to exercise reasonable care while using Academy equipment and facilities.&#13;&#10;&#13;&#10;Parents/guardians agree to reimburse the Academy for damage caused by the Student due to misuse, negligence, or failure to follow instructions.&#13;&#10;&#13;&#10;PHOTO & VIDEO RELEASE&#13;&#10;&#13;&#10;I/We grant permission for the Academy to photograph or record the Student during participation in programs and to use such images or recordings for promotional, marketing, educational, or informational purposes without compensation.&#13;&#10;&#13;&#10;Students will not be identified by full name without separate written consent.&#13;&#10;&#13;&#10;The Academy is not responsible for third-party actions on public platforms but will remove identifying content upon written request when reasonably possible.&#13;&#10;&#13;&#10;HEALTH & SAFETY POLICIES&#13;&#10;&#13;&#10;Parents/guardians agree to keep Students home if they are ill or exhibiting symptoms of illness.&#13;&#10;&#13;&#10;The Academy maintains a strict nuts-free policy. No peanut, tree nut, or nut-based products may be brought onto the premises.&#13;&#10;&#13;&#10;Parents/guardians must sign Students in and out daily unless otherwise authorized.&#13;&#10;&#13;&#10;LATE PICK-UP POLICY&#13;&#10;&#13;&#10;Students must be picked up promptly at the end of the scheduled program unless enrolled in extended care.&#13;&#10;&#13;&#10;A late fee of $10 per 10-minute interval will be assessed beginning immediately after program end time.&#13;&#10;&#13;&#10;CLASS CREDITS&#13;&#10;&#13;&#10;Class credits are non-transferable, valid only for the enrolled Student, and expire one (1) year from the purchase date.&#13;&#10;&#13;&#10;GOVERNING LAW&#13;&#10;&#13;&#10;This agreement shall be governed by and interpreted in accordance with the laws of the State of Texas. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.
