  • MUST BE SIGNED AND RECEIVED BY KWC NO LATER THAN JUNE 11, 2026

  • On behalf of myself, if 18 or older, or on behalf of myself as parent/guardian and my minor child/charge, I acknowledge and agree as follows:

    Acknowledgment of Remote Location.  I understand that Camp takes place on a remote island in Southeast Alaska accessible only by ferry or boat. Emergency response may be delayed. Medical facilities are located in a separate community and emergency transport may involve boat, aircraft, or helicopter.

    Assumption of Risk.  I acknowledge that participation includes risks including strenuous activities, wilderness hazards, wildlife encounters, watercraft activities (canoeing/kayaking), cold water immersion, transportation risks, weather exposure, illness, injury, permanent disability, or death. I voluntarily assume all such risks.

    Release of Liability.  To the fullest extent permitted under Alaska law, I release and discharge Ketchikan Wellness Coalition (KWC), Camp OdySEA, Camp Lorraine, and their directors, officers, employees, volunteers, contractors, and transportation providers (the “Released Parties”) from liability arising from participation, including ordinary negligence.

    Indemnification.  I agree to indemnify and hold harmless the Released Parties from claims arising from my or my minor child’s acts or omissions.

    Medical Consent.  I authorize emergency medical treatment and transport as deemed necessary, and for Camp or KWC to secure from a local hospital, physician and/or medical personnel any treatment deemed necessary for my or my minor child's immediate care. I agree that I will be responsible for payment of any and all medical services rendered.

    Health Insurance.  I acknowledge that Camp and KWC do not carry medical accident insurance for injuries sustained in their programs and activities. In registering myself or my minor child for Camp it is my responsibility to review our own health insurance policy for coverage. The absence of health insurance coverage does not make Camp or KWC responsible for the payment of medical expenses.

    Health Disclosure. I understand that Camp is not a therapeutic or medical treatment program.

  • I understand that Camp is in a remote outdoors environment and acknowledge that I or my child may be exposed to allergens, including food, that might cause an allergic reaction and accept that risk. In the event of an allergic reaction Camp is authorized to deny participation in certain activities.

    Acknowledgement, Acceptance and Agreement to Camp Policies.  I acknowledge receipt of Camp Policies, including a Camper Code of Contract, and agree that I or my minor child is bound by, and must comply with, the policies and statements contained therein.

    Behavior & Dismissal.  Camp reserves sole and absolute discretion to dismiss a camper for safety, behavioral concerns or failure to follow Camp Policies. Parent/Guardian is responsible for immediate pickup and all associated costs. No refund will be provided.

    Communicable Disease Acknowledgment.  I acknowledge the risk of communicable disease exposure and accept that risk and affirm that I or my minor child will comply with any public health directives.

    Transportation Disclaimer.  Camp is not responsible for airline delays, ferry interruptions, weather cancellations, or third-party transportation providers. Camp supervision of campers begins at official check-in and ends at dismissal.

    Media Release.  I give permission for my or my child’s image, voice, and name to appear in video or audio recordings, films, photographs, written articles, promotional materials, or on KWC websites, other affiliated social media platforms, and FACES International.  I understand that I will not be compensated for use nor will I seek compensation for use of my child’s image, voice, and name.  I further release my rights to ownership of any materials and promotional videos/recording produced by KWC and FACES International.  I can revoke this release at any time by submitting a written request to KWC representative.

    Force Majeure.  Camp may cancel or modify programming and/or location due to weather, natural disaster, government action, epidemic, ferry interruption, or facility damage. Liability is limited to a refund of unused camp fees.

    Binding Arbitration Agreement.  This Agreement is governed by Alaska law, and any dispute arising out of or related to this Agreement or participation in Camp shall be resolved exclusively through binding arbitration administered in Alaska under the rules of the American Arbitration Association. I waive the right to trial by jury. Arbitration shall take place in the First Judicial District of Alaska at Ketchikan. This clause survives termination of participation.
