I do hereby grant permission for my daughter/son, whose name is and hereinafter shall be referred to as “participant”, to participate in Unique Stars Performing Arts, LLC programs. In order that participants may receive the necessary medical treatment in the event of injury of illness, I hereby hold the owner and its representative(s) harmless in the exercise of this authority. I understand that Unique Stars and all affiliates with Unique Stars training programs are released from any responsibility of any injuries or accidents caused as a result of participating in Cheerleading, Gymnastics, Aerobics, Tumbling, Dance, or any other Unique Stars' Activities or Events. I further understand that Unique Stars Performing Arts, LLC and all affiliates of the Unique Stars programs and the facility are not responsible for any lost, stolen or damaged property or other mishaps, which may occur on or off the premises during the time of the program. I hereby release and forever discharge Cobb, Fulton, & Douglas Counties, Unique Stars Performing Arts, LLC and their respective officials, officer, employees, sponsors, organizers, supervisors, volunteers, participants and agents, from any and all claims, actions or causes of action of whatever kind of nature, including claims for property damage, bodily injury or death, arising out of, or sustained as a result of my/our/his/her participation in the Program(s) and all activities incidental to the program.
