Teen Track

Open to all Catholic homeschooled teens aged 13-19 while their parent(s) are in attendance at homeschool conference. Registration includes lunch. One form per teen.
Homeschooled students ages 13-19 are invited to register for Teen Track. It's a full day of activities (9 am to 3 pm). There will be many opportunities to socialize with old friends and make new friends as well. A dedicated team of volunteers will provide snacks, drinks, lunch, and dessert. (Please note: at least one parent/guardian must remain on campus attending SCCHE Conference throughout the day. Also, teens with special dietary needs are encouraged to pack their own sack lunch) Don’t miss this great opportunity to share a fun and fulfilling day with a great group of faith-filled teens!

The undersigned PARENT/LEGAL GUARDIAN (hereafter the “Parent”) hereby agrees to the following:
1. GENERAL RELEASE AND LIABILITY: Parent/Legal Guardian releases SCCHE (hereafter “SCCHE” including its officers, directors, employees, volunteers and other agents), St. Dorothy Catholic Church, and the Bishop and Diocese of Los Angeles from any liability for injuries or other damages the participating child/youth suffers arising out of or connected with the youth’s participation and attendance at the SCCHE High School Track.
2. FIRST AID: Parent/Legal Guardian hereby grants SCCHE permission to give or obtain for the participating youth emergency first aid treatment and any other treatment reasonably necessary under the circumstances. This includes permission to engage professional assistance and treatment, such as ambulance, urgent care, hospital, and physician care, all at the undersigned’s cost and expense. In such event SCCHE will make reasonable efforts to contact the parent/legal guardian or other emergency contact person listed.

  I, {parentlegalGuardian143}, the undersigned and parent/legal guardian of {teenName}, give permission for my teen to attend the SCCHE High School Track exclusively for Catholic home-school high school students at St. Dorothy Catholic Church on June 27, 2025, from 9:00 a.m. - 3:00 p.m. I will review the Student Agreement (below) with my teen, and I understand if my teen is found to have any alcoholic beverages, drugs, or weapons, or is not following behavior standards in the Agreement, I will be notified and make arrangements to have my teen picked up immediately.
