WAIVER OF LIABILITY
Event: The Laundry Room – Premier Event
Date: March 28th, 2026
Location: 512 Maple Ave W, Vienna, VA
The Lab DC LLC d/b/a ALL10 Breaking (hereafter collectively, “ALL10 Breaking”) is permitting the participant to attend and participate in dance battles, sessions, rehearsals, performances, and events (in person and online), including but not limited to The Laundry Room (Premier Event). By signing below, Participant voluntarily indemnifies, releases from liability, and holds harmless ALL10 Breaking, its owners, agents, employees, volunteers, sponsors, independent contractors, personnel, event organizers, facilities used by ALL10 Breaking, including but not limited to 512 Maple Ave W, Vienna, VA, and any of these parties’ insurance companies (collectively, the “Released Parties”), from any accident, injury, illness, death, loss, or damage to person or property arising directly or indirectly from Participant’s participation in the event.
In the event Participant is injured, Participant agrees to assume all financial responsibility for any medical costs incurred, whether through personal health insurance or otherwise. The Released Parties assume no responsibility for any medical expenses, injury, or damage suffered in connection with participation in the event or use of the premises.
ASSUMPTION OF RISK
PARTICIPANT EXPRESSLY ASSUMES ALL RISK OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE, AND AGREES TO RELEASE AND HOLD HARMLESS THE RELEASED PARTIES FROM ANY AND ALL LIABILITY, INCLUDING CLAIMS ARISING FROM NEGLIGENCE.
Participant agrees not to bring any claim, lawsuit, or action against the Released Parties for any loss or damage related to participation. Participant acknowledges the inherent risks associated with physical dance activity and participation in live events and signs this waiver of their own free will.
CERTIFICATION OF HEALTH
I certify that I am physically and mentally able to participate in or attend the event. I agree to stop participation if my condition changes and acknowledge responsibility to disclose any medical condition that may affect safe participation. I assume all risks related to known or unknown medical conditions.
MINORS
If signing on behalf of a minor, I certify that I am the parent or legal guardian and agree to all terms on the minor’s behalf. I waive and release any claims the minor or another parent/guardian may have against the Released Parties and agree to indemnify the Released Parties against such claims.
MEDIA RELEASE
I grant ALL10 Breaking permission to photograph, record, and use my likeness, image, or voice captured during the event for promotional, marketing, and archival purposes in any medium, without compensation, in perpetuity.
RULES & CONDUCT
I agree to follow all rules, instructions, and guidelines set by ALL10 Breaking and event staff. Failure to comply may result in removal from the event without refund.
PERMISSION TO RENDER EMERGENCY AID
I understand that ALL10 Breaking staff are not medical professionals. In the event of injury or illness, I authorize staff to administer basic first aid and/or seek emergency medical assistance, including calling 911. I authorize licensed medical professionals to provide emergency care if needed and understand reasonable efforts will be made to notify me or a guardian.
ELECTRONIC SIGNATURE CONSENT
By signing electronically, I consent to the use of an electronic signature in place of a handwritten signature. I understand I may request a paper copy at any time without penalty.
By signing below, I acknowledge that I have read, understand, and agree to all terms of this Waiver of Liability.
