EMERGENCY MEDICAL AUTHORIZATION
I authorize camp staff and volunteers to provide basic first aid and to obtain emergency medical treatment for my child if I cannot be reached. I understand that reasonable efforts will be made to contact me immediately in the event of an emergency. I give permission for my child to be transported to a medical facility and treated by licensed medical personnel if necessary.
ASSUMPTION OF RISK & LIABILITY WAIVER
I understand that participation in youth camp activities includes normal physical activity and inherent risks, including but not limited to outdoor play, sports, recreational activities, and group games. I voluntarily allow my child to participate and acknowledge these risks. I agree to hold the camp, its organizers, staff, and volunteers harmless from liability for injuries or accidents that may occur during normal participation, except in cases of negligence.
PARTICIPATION & ACTIVITY PERMISSION
I give permission for my child to participate in all supervised camp activities, which may include indoor and outdoor recreation, sports, games, and group activities.
PHOTO & MEDIA RELEASE
I grant permission for photographs or videos of my child taken during camp activities to be used for camp or program-related purposes, including newsletters, promotional materials, websites, or social media.
