Assumption of Risk I understand that cheerleading, tumbling, stunting, jumping, conditioning, and related activities involve inherent risks, including but not limited to: falls, collisions, sprains, strains, fractures, concussions, and other injuries.

I acknowledge that these risks may occur during practices, competitions, performances, travel, and any Program‑related events.

I voluntarily allow my child to participate with full knowledge of these risks. Release of Liability In consideration for my child’s participation, I hereby release, waive, and hold harmless Silent Strike Cheer, its owners, coaches, staff, volunteers, representatives, and affiliated facilities from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur while participating in the Program, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Medical Authorization I authorize Silent Strike Cheer staff to obtain emergency medical care for my child if necessary. I understand that I am responsible for all medical expenses resulting from any injury or illness that occurs during Program activities. Participant Responsibility I agree that my child will follow all safety rules, instructions, and guidelines provided by coaches and staff. I understand that failure to follow safety expectations may result in removal from activities.

I certify that I am the parent or legal guardian of the participant and that I have read, understood, and voluntarily agree to this Waiver and Release of Liability.
