Sports Waiver
1. Assumption of Risk
I understand that football and related activities involve inherent risks, including but not limited to
physical injury, illness, accidents, or other harm. I voluntarily choose to participate and accept all risks
associated with these activities.
2. Release of Liability
I hereby release, waive, and hold harmless Trinity College of Florida, its coaches, staff, employees,
volunteers, and affiliates from any and all claims, liabilities, damages, or expenses arising from injury,
illness, or loss sustained while participating in TCF football activities.
3. Medical Treatment
I authorize TCF staff or representatives to seek emergency medical care on my behalf if needed. I
understand that I am responsible for all medical costs related to injury or illness.
4. Insurance Responsibility
I acknowledge that I am personally responsible for maintaining my own medical/health insurance, and
that TCF is not liable for costs related to injury or treatment.
5. Fitness to Participate
I confirm that I am physically able to participate in football-related activities and have disclosed any
medical conditions that may affect my participation.
