Official registration for Midwest Elite Circuit (MEC). Completion and payment are required for roster consideration. All participants must agree to the liability waiver and media rights agreement below.

Assumption of Risk & Release of Liability
I acknowledge that participation in basketball activities involves inherent risks including but not limited to bodily injury, concussion, permanent disability, or death.

I voluntarily assume all risks associated with participation in Midwest Elite Circuit activities, including practices, games, travel, and related events.

I release, waive, and discharge Midwest Elite Circuit, its owners, directors, staff, coaches, facility partners, sponsors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising from participation.

I certify that the participant is physically fit and has no medical condition preventing participation.

I understand that Midwest Elite Circuit does not provide medical insurance coverage for participants.
I Agree to the Assumption of Risk & Liability Release
Media & Live Streaming Authorization
I grant Midwest Elite Circuit, its affiliates, sponsors, and media partners the irrevocable right to record, film, photograph, livestream, and otherwise capture the participant’s name, image, likeness, voice, statistics, and performance.

This includes but is not limited to:
• Live-stream broadcasts
• YouTube, Instagram, Twitch, and other social platforms
• Promotional materials
• Highlight reels
• Advertising campaigns
• Rankings publication
• Website and social media use

I understand that no compensation will be provided for such use.

This authorization is granted in perpetuity and may be used for commercial, promotional, advertising, or branding purposes.

Advertising & Sponsor Integration Consent
I understand that Midwest Elite Circuit may incorporate sponsor logos, brand placements, advertisements, and promotional integrations within livestreams, digital media, uniforms, or event materials.

I acknowledge that participant footage and images may appear alongside sponsor branding.

Application, Registration & Refund Policy
Submitting your application to the Midwest Elite Circuit does not guarantee you a spot; the MEC board will review all applications. If approved, you will receive your formal invitation via email.
Registration Fee: $225 (Full Payment Required)

All registrations are final once teams are announced.

Refunds will only be issued if:

• The season is cancelled

• The participant sustains a verified medical injury prior to Week 1

No refunds will be issued for:

• Missed games

• Schedule conflicts

• Voluntary withdrawal
