  • Summer Camp Fees:

     

    Pay in Full: $600 if paid by 5/15/25

    Weekly Rate: $75.00 per week

    • Late Fee: Parents are due on Monday's by 7:00am late paymemts will be charged at $25.00 late fee

    Monthly Rate: 300.00 per month due on the 1st of of the monthly of June and July 

    • Late Fee: Payments are due the 1st day of the month.  Payments made after the 2nd day of the month will be charged a $25.00 late fee.

     

    Late Pick Up Fees: Late pick up fees start at 5:46 and are $1.00 per minute. This late fee must be paid no later than the follwoing day or before the child returns.


  • Liability Waiver: In consideration of your acceptance of the enrollment, I, intending to be legally bound, do hereby waive, release, and forever discharge all rights and claims against Crystal’s World of DANCE, and/or involved staff for damages or injury sustained by me or my child while in classes or on studio premises before or after class, or at any other studio activities including but not limited to recitals, workshops, camps, intensives, competitions, performances, and visits to surrounding community businesses. I authorize and agree to hold harmless Crystal’s World of DANCE, to obtain medical emergency assistance when they deem necessary and for Crystal’s World of DANCE, to provide transportation to receive such medical assistance for myself and/or my child named below. I further agree to be financially responsible for all medical services provided to me and/or my child named below, including the cost of defense and enforcement of this indemnity agreement. I understand and agree that Crystal’s World of DANCE and staff may administer first aid on Crystal’s World of DANCE premises in the event of minor injuries. Guardians will be notified if basic first aid is provided to any child. I authorize Crystal’s World of DANCE, to use photos taken of me or my child while at dance or dance functions for marketing and promotional materials, including website. I acknowledge that my child may be videotaped for educational and performance purposes only, including but not limited to recital(s).Please note that Crystal’s World of DANCE, agrees to not make public or resell any private information provided to it by students and their families. I have read the foregoing Studio Waiver and agree with it in all respects. I acknowledge that I have READ, UNDERSTAND, and AGREE TO ABIDE BY all “Crystal’s World of DANCE Studio Policies” and all Registration, Tuition and Recital Policies”.   • This signature is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releases and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releases for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law

  • WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19

    Mask are required and must be worn by all campers at all times other than when eating or swimming. 

    ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT
    In consideration of being allowed to participate on behalf of Crystal’s World of Dance and or Project C.A.M.P and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

    Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
    I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,I , for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors ofpremises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

  • Medical Release and Authorization

    As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

    Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

    Permission is also granted to the Project CAMP and Crystal's World of Dance . and its affiliates including Directors, Teachers, and Staff and approved Volunterrs to provide the needed emergency treatment prior to the child’s admission to the medical facility.

    Release authorized on the dates and/or duration of the registered season.

    This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

    Summer Camp Parent Policies & Procedures
     

    Welcome to Summer Camp at Crystal’s World of Dance! Our goal is to provide a safe, structured, fun, and enriching environment for every camper. Please read all policies carefully.

     

    Campus Safety & Access Policy

     

    No unauthorized individuals are permitted on campus.
    Parents, family members, and visitors are not allowed inside the building during camp hours.
    Entry is restricted to approved staff and authorized personnel only.
    All staff must meet state compliance standards and hold Level 2 Background Screenings.
     

    Level 2 Background Screening & Chaperone Policy

    All staff working with children are required to have an approved Level 2 Background Screening.
    Only individuals with Level 2 Background Screenings may chaperone field trips.
    No exceptions will be made.
    Background screenings must be completed and cleared prior to participation.
     

    Field Trip Policy
    Field trips are an important part of camp fun and learning:

     Parents will receive advance written notice of all trips.
    Signed permission slips are required for participation. Campers must wear designated camp shirts if applicable.
    Students must follow all behavior expectations while off campus.
     
    Chaperone Requirements:

    Only individuals with Level 2 Background Screening may chaperone.
    Chaperones must follow all studio rules and staff directions.
    Siblings or additional guests are not permitted on trips.
     

    Failure to follow behavior guidelines may result in suspension from future activities.

     


    Drop-Off & Pick-Up Policy
     

     Camp Hours:

    Monday – Friday

    Drop-Off: 7:30 AM – 8:30 AM
    Camp Begins: 8:30 AM
    Pick-Up: 5:00 PM – 5:45 PM
     

     

    Drop-Off Procedures
     

    Parents should pull up into the car rider drop-off area.
    Students must remain in the car until staff assists them.
    Breakfast is available daily starting at 8:30 AM.
    Do not leave students unattended outside the building.
     

    Pick-Up Procedures. Parents should pull up into the car rider pick-up area.
    Staff will bring your camper to the car.
    Campers will only be released to individuals listed on the authorized pick-up list.
    A valid photo ID may be required.
    Pick-up closes at 5:45 PM.
    Late Fee: $2 per minute after 5:45 PM. The late fee must be paid before your child can return to camp.
     

     


     

    Early Pick-Up
     

    Park and come to the front entrance.
    Notify staff in advance via phone or GroupMe if possible.
    Allow time for your camper to transition safely from activities. Parents must park and pick up at the front door.
     

     

    Parent Visiting Policy
     

    Parents are not permitted to remain inside the building during camp hours.
    Classroom observation is not allowed.
    Special performance days or showcases will be announced in advance.
     

     

    Dress Code Policy

    Campers are required to wear the official dance dress code:

    Solid black dance leotard
    Skin-tone dance tights
    Skin-tone ballet shoes
    Skin-tone jazz shoes
    Hair must be secured neatly away from the face (ballet bun preferred)
     

     


    Additional Guidelines:
    No open-toe shoes, or sandals
    Minimal jewelry
     

    Campers not in proper attire may sit out of technique classes until dressed appropriately.

    Medication Policy
     

    Crystal’s World of Dance staff are not authorized to distribute any medication.
    Students who require medication must self-administer under supervision if needed.
    Students with asthma must have an extra labeled inhaler stored at the studio.
    All medications must be clearly labeled with the student’s name.
     

     

    Health & Illness Policy
     

    Campers must be symptom-free for 24 hours before attending camp.
    If a child becomes ill during camp, parents will be contacted immediately for pick-up.
    Minor injuries will be treated on-site; parents will be notified as necessary.
     

     


    Behavior Expectations

    Campers are expected to:

    Follow directions
    Show respect to staff and peers
    Participate positively
    Keep hands and feet to themselves
     

     


    Disciplinary Steps:

    Verbal warning
    Parent notification
    Possible dismissal from camp (no refund)
     

     

    Meals & Snacks
     

    Breakfast is provided starting at 8:30 AM.
    Campers should bring nut-free lunches and snacks if attending full-day camp.
    No food sharing is permitted.  e are not allowed to heat up food for students.
     

    Payments & RefundsTuition must be paid in full before the first day of camp.
    Registration fees are non-refundable.
    Missed days are non-refundable.
    Emergency refunds will be considered case-by-case.
     

    Media Release

    Photos and videos may be taken for promotional purposes.
    Written notice must be submitted if you decline media participation.

    Summer Camp 2026 Supply List

    Please label all items with your child’s name

    ALL CAMPERS MUST BRING A BOOKBAG DAILY WITH THE FOLLOWING ITEMS

    1.       Dancewear: Solid Black Dance Leotard, Skin Tone Dance Tights, and  Skin Tone (Brown/ Nude) Ballet Shoes

    2.       One solid white t-shirt your child’s size (needed by 6/10 for a craft project)

    3.       One pack of crayons

    4.       A hat for outdoors (or whatever your child uses to avoid the sun))

    5.       A bottle of Deodorant (personal care items)

    6.       A bottle of sunscreen (or whatever your child uses to avoid sunburn)

    7.       A refillable water bottle.,

    8.       A one piece bathing suit and towel ( 2 piece bathing suots are nt allowed)

    9.       Hand sanitizer

    10.     One pack of baby wipes or personal cleansing wipes
