Indemnity Form for Player/Participant - Valkyrie Hockey

THIS DOCUMENT REPRESENTS A RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND AN INDEMNITY AGREEMENT.
BY SIGNING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE, OR CLAIM COMPENSATION FOLLOWING AN ACCIDENT.

ASSUMPTION OF RISK
I am aware that participation in physical activities, such as practicing hockey skills, power skating, off-ice training, and hockey games – hereinafter collectively referred to as “HOCKEY ACTIVITIES” – has inherent risks, including but not limited to:
Injury, including death, resulting from physical exertion related “HOCKEY ACTIVITIES”
Risk of injury from using equipment such as pucks, sticks, weights, cones, and pads.
Loss or damage of personal belongings.
Negligence or criminal acts of non-Valkyrie Hockey personal.

I freely accept and fully assume all such risks, dangers, hazards, such as the possibility of personal injury, death, or the damage/loss of property, pertaining to participating in the aforementioned “HOCKEY ACTIVITIES.”

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT

In acknowledgement of the above stated risks associated with participation in ”HOCKEY ACTIVITIES,” I hereby agree to the following:

1.TO WAIVE ANY AND ALL LIABILITY CLAIMS that I have or may have in the future against Valkyrie Hockey by Greer Harris, its proprietors, governors, officers, employees, students, agents, volunteers and independent contractors (all of whom are hereinafter collectively referred to as the Releasees).
2. TO RELEASE THE RELEASEES from any liability involving the loss or damage of property, the loss of life, ANY physical injury or any expenses that the participant may suffer as a result of participation in “HOCKEY ACTIVITIES” (including, but not limited to, negligence, breach of contract, or breach of any statutory or other duty of care, including failure on the part of the releasees to take reasonable steps to safeguard or protect the participant from the risks, dangers and hazards of participating in “HOCKEY ACTIVITIES”).
3. TO INDEMNIFY THE RELEASEES from any and all liability for any damage to the property of, or personal injury to, any third party or next of kin, including death, resulting from my participation in “HOCKEY ACTIVITIES”.
4. THIS AGREEMENT SHALL BE EFFECTIVE AND BINDING upon the participant`s heirs, next of kin, executors, administrators, and representatives in the event of the participant`s death or incapacitation.

In entering into this Agreement, I am not relying upon any oral or written representations or statements made by the Releasees other than what is set forth in this Agreement.

I HAVE READ AND UNDERSTOOD THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS AGAINST THE RELEASEES, INCLUDING THE RIGHT TO SUE, OR CLAIM COMPENSATION FOLLOWING AN ACCIDENT.
I hereby approve my child’s participation in the Valkyrie Hockey 2026 Summer Camp. I certify that my child is in good health and able to participate in the activities listed on the websites camp description. I have listed any allergies or known health conditions in the field provided above. I also certify that I have read and agree to the Indemnity Form.
