Please fill out the form to reserve your spot for the meeting and make the appropriate payment for the meal provided. (* = required field). Dinner includes Salisbury Steak, Mashed potatoes w/mushroom gravy, Green Beans w/bacon & onions, Pineapple Fluff, Dinner Rolls, Drinks. Chef salad alternative is available.
