Informed Consent and Acknowledgement

By continuing with this booking, I hereby provide my consent for my child's participation in all activities organised by the Future Stars Camps during the chosen camp session. By accepting my child's participation, I acknowledge and accept all the potential risks and hazards associated with these activities. I release, exempt, and indemnify the camp and all its officials, representatives, and agents from any responsibility for injuries that may occur to my child during their engagement in the camp sessions.

If my child sustains an injury, I waive any claims against Future Stars Camps, including its coaches, affiliates, fellow participants, supporting organizations, advertisers, and, if applicable, the owners and landlords of the premises where the event is held. 

Medical Release and Authorization

By continuing with this booking , I confirm that the information I have provided regarding my childs medical needs, conditions, allergies, and medication is accurate and up to date. I give permission for my child to receive first aid and/or emergency medical treatment if required while taking part in sessions. I understand that every reasonable effort will be made to contact me in an emergency. I agree that the organisers cannot be held responsible for any medical costs incurred as a result of treatment. 

I confirm my child is toilet trained and is able to use the facilities appropriately.
