Event Name: The Fatherhood Forge Workshop & Community Block Party

Date: Saturday, June 20, 2026

Location: Oak Hill Center & Oak Hill Field - 2805 Metropolitan Pkwy SW, Atlanta, GA 30315

Participant Event Hours: 11:00 AM – 3:00 PM

Setup & Breakdown: 9:30 AM – 3:30 PM

Volunteer Arrival & Briefing:
Volunteers must arrive between 9:45 AM and 10:00 AM.
A mandatory volunteer briefing will begin promptly at 10:00 AM.
Volunteers are expected to remain on-site until the end of their assigned shift.

Volunteer Responsibilities May Include:
• Registration support
• Workshop assistance / Child Care
• Kids’ zone supervision
• Field activity support
• Vendor and participant guidance

 

1. Consent to Photography, Video, and Audio Recording

I understand and agree that during the Fatherhood Forge event, Peach Rebel, LLC and its event partners may capture photographs, video footage, and audio recordings that may include my image, likeness, voice, participation, booth, products, or activities.

I voluntarily grant permission for this media to be collected.

 

2. Rights Granted

I authorize Peach Rebel, LLC and all event partners to:

Use, reproduce, publish, and distribute the media captured
Edit, modify, or adapt the media
Use the media in print, digital, broadcast, and social media formats
Use the media for promotional, marketing, educational, and archival purposes
Use the media without restriction as to time, location, or frequency
I understand that all media captured becomes the property of Ani Vision, LLC and may be used without further approval or compensation.

 

3. No Compensation

I acknowledge that:

I will not receive payment, royalties, or financial benefits for the use of my image, likeness, voice, or participation.
Volunteers and participants are not compensated for their involvement in the event.
Vendors understand that media featuring their booth or products may be used without compensation.
 

4. No Liability

I release Peach Rebel, LLC, its staff, volunteers, event partners, sponsors, vendors, contractors, and the venue from any claims or demands related to:

The capturing of media
The use or distribution of media
Any distortion, alteration, or composite use of the media
Any claims of invasion of privacy, defamation, or misrepresentation
This release applies to all media captured before, during, and after the event.

 

5. Vendor‑Specific Acknowledgment (if applicable)

If I am participating as a vendor, I understand that:

Media may include my booth, products, signage, and interactions with attendees.
Peach Rebel, LLC may use this media for event promotion or documentation.
I grant permission for such use without limitation.
 

6. Minor Release (if applicable)

If I am signing on behalf of a minor, I certify that:

I am the parent or legal guardian of the minor(s) listed below.
I grant full permission for their image, voice, and participation to be recorded and used as described above.
 

7. Agreement & Signature

By signing below, I acknowledge that I have read, understood, and agree to the terms of this Media Release Agreement.

1. Assumption of Risk

I understand that participation in the Fatherhood Forge event may involve physical activities, hands‑on workshops, outdoor challenges, movement, and interaction with tools, equipment, and other participants. I acknowledge that these activities carry inherent risks, including but not limited to:

Minor injuries (scrapes, cuts, bruises)
Physical strain or discomfort
Accidental contact with tools or equipment
Weather‑related exposure
Slips, trips, or falls
I voluntarily choose to participate (or allow my child/children to participate) and accept full responsibility for any risks associated with the event.

 

2. Release of Liability

By signing this waiver, I agree that Ani Vision, LLC, its staff, volunteers, event partners, sponsors, vendors, contractors, and the venue/venue owners are not liable for any injury, accident, loss, or damage that may occur during or as a result of my participation in the event.

I release and hold harmless all event hosts and partners from any and all claims, demands, or causes of action arising out of participation in the Fatherhood Forge event, whether caused by negligence or otherwise, to the fullest extent permitted by law.

 

3. Medical Authorization

In the event of an emergency, I authorize event staff or volunteers to secure medical treatment for myself or my child(ren). I understand that I am solely responsible for any medical expenses incurred.

 

4. Personal Property

I understand that I am responsible for my own personal belongings and equipment. Ani Vision, LLC and event partners are not responsible for lost, stolen, or damaged items.

 

5. Vendor‑Specific Acknowledgment (if applicable)

If I am participating as a vendor, I acknowledge and agree that:

I am responsible for the setup, operation, and breakdown of my booth.
I will comply with all safety guidelines provided by the event team and venue.
I assume full responsibility for the safety of my products, equipment, and materials.
Ani Vision, LLC and event partners are not liable for any incidents involving my booth, products, or interactions with attendees.
 

6. Volunteer‑Specific Acknowledgment (if applicable)

If I am participating as a volunteer, I acknowledge and agree that:

I am offering my time voluntarily and will not receive financial compensation.
I will follow all safety instructions and event guidelines.
I understand that I may be assigned to physical tasks and will communicate any limitations to event leadership.
 

7. Media Release

I grant permission for Ani Vision, LLC and event partners to capture photos, video, and media content of myself (and/or my child/children) during the event for promotional, marketing, and archival purposes.

 

8. Agreement & Signature

By signing below, I acknowledge that I have read, understood, and agree to the terms of this Media Release, Safety & Liability Waiver.
