WAIVER AND RELEASE OF LIABILITY
1. Assumption of Risk
I understand that participation in dance, acrobatics (acro), tumbling, conditioning, and related physical activities involves inherent risks, including but not limited to falls, collisions, muscle strains, sprains, fractures, head injuries, and other serious injuries.
I voluntarily allow my child to participate in Summer Dance & Acro classes and assume all risks associated with such participation.
2. Release and Waiver of Liability
I, on behalf of myself and my child, hereby release and hold harmless [Studio Name], its owners, instructors, staff, volunteers, and affiliates from any and all claims, demands, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur during participation in classes or while on studio premises, except in cases of gross negligence or willful misconduct.
3. Medical Authorization
I certify that my child is physically fit to participate in dance and acro activities. I understand it is my responsibility to disclose any medical conditions, injuries, allergies, or limitations that may affect participation.
In the event of an emergency, I authorize the studio to obtain medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
4. Photography & Media Release (Optional – Modify as Needed)
I grant permission for my child to be photographed or recorded during classes for studio promotional purposes, including social media, website, and marketing materials.
5. Code of Conduct
I understand that respectful behavior is expected from all students and parents. The studio reserves the right to dismiss any participant for unsafe or disruptive behavior without refund.
6. Acknowledgment
I have carefully read this waiver and fully understand its contents. I understand that by signing below, I am giving up certain legal rights on behalf of myself and my child.
