    Camp Fee of $150 includes a camp t-shirt, give aways and a college or professional athlete as a guest speaker (email info@alphaomegasports.org to request a sibling discount).   • Informed Consent and Acknowledgement

    I hereby give my approval for my child’s participation in any and all activities prepared by Alpha & Omega Sports, Inc., during the selected camp. In exchange for the acceptance of said child’s candidacy by this organization. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Alpha&Omega Sports, all its respective officers, agents, volunteers and representatives from any and all liability for injuries to the said child arising out of traveling to, participating in, or returning from selected camp sessions.

    In case of injury to said child, I hereby waive all claims against Alpha&Omega Sports, Inc., including all coaches and affiliates, all volunteers, participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball, soccer and flag football.

    Cancellation Policy

    A full refund (minus $25 administrative fee) will be given for cancellations made thirty days or more before the camp start date. Cancellations made 21 to 29 days before the camp start date are eligible for a $75 refund. For all cancellations within 20 days of the camp start date, all payments are non-refundable.
