PSA Summer 2026 Camp Registration

June 6-7th at Ackerman Sports & Fitness Center in Glen Ellyn.

Schedule:

Session 1: 9:00-11:50am (incoming 3rd - 5th graders)
Session 2: 12:00-2:50pm (incoming 6th - 8th graders)

 

Fee: $149.99 for both days OR $89.99 for one day

Important Notes:
-Each camper is responsible for bringing their own basketball (include name on the ball)
-No refunds once we finalize camp enrollment with Ackerman after May 1st

-Refunds will not be 100%, as there is a $0.30 + 2.9% fee for all charges. This fee will not be deducted from your refund

-There will be time on Day 2 for autographs and pictures. Please bring a sharpie if you would like an autograph!

Parental Consent

I the parent or legal guardian of the camp participant named above, do hereby grant permission for his/her
participation in any and all training/camp/clinic activities a part of Pierce Sports Academy (PSA).
Release from Liability, I agree to assume all risks and hazards incidental to participation in this camp/clinic. I do hereby waive, release, absolve, indemnify, and agree to hold harmless PSA, its officers, directors, coaches, sponsors, volunteers, employees, participants, affiliates, and representatives, for any claim arising out of an injury to my child, whether the result of negligence or any other cause.

Furthermore, I waive, release, remise, covenant not to sue, and fully discharge PSA, its officers, directors, coaches, sponsors, volunteers, employees, participants, affiliates, and representatives of any, liabilities, demands, actions or rights of action, damages of any kind (Causes of Action), whatsoever, related to or arising out, or in any way connected to participation in PSA, including those
Causes of Action allegedly from, or in any way related to, the negligent acts or omissions of PSA, its officers, agents, and or employees.

I certify that the participant has no known medical problems that would increase the risk of illness, injury, and/or death, as a result of participation in PSA, or any other activity facilitated and or designed by PSA.

Medical Release and Authorization

Because my child is involved in an active training and conditioning camp/clinic, I understand that there may be an occasion when an injury occurs that requires medical treatment and representatives of PSA are unable to contact me. This situation may occur before, during or after the camp/clinic activities, while at the program site.

I hereby grant permission to PSA to administer first aid, secure proper treatment, and/or hospitalize my (son, daughter, ward) in case of emergency, provided they are unable to communicate with me, and according to their best judgment.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
