Acknowledgment of Risk & Waiver of Liability
 
I, the undersigned, acknowledge that participation in the Fierce Beauties Football League tryouts involves physical activity and carries certain inherent risks, including but not limited to injury, illness, or accidents.

 
By signing this form:

I voluntarily assume all risks associated with my/my child’s participation in tryouts, practices, and related activities.

I release and hold harmless Fierce Beauties Football League, its directors, coaches, volunteers, sponsors, and affiliates from any liability, claims, or expenses arising from participation.

I certify that I/my child is physically fit to participate and that any medical conditions have been disclosed to league staff.

I authorize staff or volunteers to obtain emergency medical care if necessary, with the understanding that I am responsible for all associated costs.
