By submitting my enrolment, I acknowledge that participation in Artz on Show workshops and activities involves physical movement and group-based activities which carry an inherent risk of injury. I confirm that my child is physically and medically fit to participate, and I have disclosed any relevant medical conditions, allergies, learning needs, or behavioural considerations on this enrolment form. I understand that Artz on Show and its staff will take all reasonable steps to provide a safe and supportive environment for participants. However, I accept that accidents may occur and agree that Artz on Show, its staff, contractors, and venue providers will not be held liable for injury, loss, or damage except where caused by proven negligence. I consent to my child participating in all workshop activities and, in the event of illness or injury, authorise Artz on Show staff to seek medical treatment as deemed necessary. I understand that any medical or associated costs remain my responsibility.I agree to support Artz on Show’s health and safety rules, behavioural expectations, and staff instructions at all times.By submitting this enrolment, I confirm that I have read, understood, and accepted this Health & Safety acknowledgement on behalf of my child.
