I hereby certify that all information provided in this registration/application is true, accurate, and completed to the best of my knowledge. I understand that any false statement, misrepresentation, or omission of material information may result in denial of participation or immediate dismissal from camp without refund. On behalf of myself and/or my minor child, I voluntarily assume all risks associated with participation in camp activities. I hereby release, waive, and discharge the camp, its sponsoring organization, directors, officers, employees, volunteers, and representatives from any and all claims, liabilities, damages, or causes of action arising out of or related to participation in camp activities, except in cases of gross negligence or willful misconduct. I understand that reasonable efforts will be made to contact the parent/guardian listed on this form prior to treatment when possible. I accept financial responsibility for any medical expenses incurred. I acknowledge that campers are expected to follow all camp rules and behavioral standards. The camp reserves the right to dismiss any participant whose behavior is deemed unsafe, disruptive, or inappropriate. No refund will be issued in the event of dismissal. I grant permission for the camp and its affiliated organization to photograph, video record, and/or use images or likenesses of my child for promotional, educational, or ministry purposes in print, digital, or social media formats without compensation.
I further acknowledge that the camp reserves the right to verify any information submitted and to make all participation decisions in its sole discretion, in the best interest of the camp and its participants.
