Parental Consent and Liability Waiver
In consideration of the 2026 Cowichan Capitals ID Camp allowing me to participate, I agree and understand the following:
Cowichan Capitals reserve the right to reschedule or regroup players according to enrolment and team balance. Cowichan Capitals and The Easta Calgary Twin Rinks and employees will not be liable or held responsible for any accidents or loss however caused and agreed to release all mentioned parties from all claims and damages. I further verify that my child or I have no medical problems and is in good physical health and the above-mentioned parties will not be held responsible for any medical, dental or insurance claims resulting from injury. Any medical conditions will be communicated prior to Capitals Staff.
