Camp Participation & Medical Treatment
I grant permission for my child(ren) listed above to participate in Trinity Academy Summer Camp. In the event of a medical emergency, I authorize camp staff and/or school employees to act on my behalf in providing, arranging and consenting to medical care. I acknowledge that participation in this activity involves a degree of risk, and I release and hold harmless Trinity Academy, its board members, employees, and volunteers from any liability in the event of an accident or injury to my child.*
