Camp Participation & Medical Treatment
I grant permission for my child(ren) listed above to participate in Trinity Academy Summer Camp. In the event of a medical emergency, I authorize camp staff and/or school employees to act on my behalf in providing, arranging and consenting to medical care. I acknowledge that participation in this activity involves a degree of risk, and I release and hold harmless Trinity Academy, its board members, employees, and volunteers from any liability in the event of an accident or injury to my child.
In the event of illness or injury, I authorize Trinity Academy to seek medical treatment for my child when I or the listed guardians cannot be reached. I give permission for hospital staff and licensed medical personnel to provide necessary emergency treatment. I understand that Trinity Academy will make every effort to contact me immediately. I also acknowledge that participation in camp activities involves a degree of risk, and I release and hold harmless Trinity Academy, its board members, employees, and volunteers from any liability in the event of an accident or injury to my child.
Media & Photography Release
Trinity Academy may take photos or videos during camp activities to celebrate and share the camp experience. I give permission for my child to be photographed and/or recorded and for these images to be used for school-related purposes such as newsletters, the website, social media, or promotional materials. I understand that no names or identifying information will be used without additional consent.
