Informed Consent, Assumption of Risk & Liability Waiver
I, the undersigned parent/guardian, give permission for my child to participate in NJ Pride FC soccer classes.
I understand that participation in physical activities, including soccer, involves some inherent risks such as minor sprains, strains, or falls. While NJ Pride FC takes every reasonable precaution to ensure a safe environment, I acknowledge that injuries can occur.
I voluntarily assume these risks and agree that NJ Pride FC, its coaches, staff, and affiliates shall not be held responsible for injuries or accidents that may occur during normal participation, except in cases of gross negligence or misconduct.
In the event of an emergency, I authorize NJ Pride FC staff to seek appropriate medical care for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
I also acknowledge that NJ Pride FC may take photographs or videos during sessions for promotional purposes. If I do not consent to this, I will notify NJ Pride FC in writing.
This agreement is governed by the laws of the State of New Jersey. If any portion of this agreement is found to be unenforceable, the remaining provisions shall remain in full effect.
