Terms and Conditions:
By submitting this form, I acknowledge that participating in LQ's Performing Arts Center's Hip-Hop Dance Class on February 12, 2026, involves physical activity, and I hereby release this non-profit organization, the physical dance studio, and its owners/instructors from any liability for injuries or accidents that may occur during classes or events.
I further grant permission to LQ's Performing Arts Center to take photographs and/or video recordings of the participant during the dance class. I understand that these images and recordings may be used for promotional, marketing, social media, website, or other educational and publicity purposes without compensation. I waive any right to inspect or approve the finished product in which my likeness appears and release LQ's Performing Arts Center from any claims related to the use of these materials.
By completing this registration form, the participant acknowledges that a payment of $10 per class is required for the cost of the class. Upon submission of the registration, a payment link will become active. The payment must be rendered in full before the participant is eligible to attend the class. Failure to complete the payment prior to the class date/time will result in forfeiture of the participant's spot.
Once you complete the registration form and pay the fee, you will receive text and email messages within 24 hours containing the location of the dance class.
