I hereby authorize the staff of the Oak Mountain Academy Summer Camps 2026 to act for me in accordance with their best judgment in any emergency requiring medical attention for my child(ren). I hereby waive and release the school and its employees from any and all liability for any injuries incurred while at camp. I have no knowledge of any physical impairment that would be affected by the registered child(ren)’s participation in the school program as outlined.
