Waiver & Release of Liability
In consideration of being allowed to participate in the above described workshop, I, the undersigned, acknowledge, appreciate and agree that:
Assumption of risk
I understand that participation in the workshop involves inherent risks, including but not limited to personal injury, emotional distress, illness or death. I am aware of my own physical limitations and will stop or modify my activity before becoming injured or aggravating a pre-existing injury. I voluntarily assume all such risks,injuries, or damages, both known and unknown, and accept full responsibility for my participation.
Release from liability
I, for myself and on behalf of my heirs, assigns and personal representative and next of kin, hereby release, indemnify and hold harmless the facilitators of this workshop, other participants, and owner of the above listed location for the workshop from any and all claims.
