★All persons picking up children must be 18 years of age or older and provide identification. Children must be signed in and out of the recreation program daily.

★Any special instructions such as custody/restraining orders must be attached to this registration and discussed personally with the program directors. All information will be kept confidential.

Camp On The Move, Adventure Awaits Recreation Program operates Monday through Friday 9:00am to 3:30pm

★ $50 DEPOSIT IS DUE AT THE TIME OF REGISTRATION. THE DEPOSIT WILL GO TOWARDS TUITION

 REGISTRATION DEPOSIT IS NON-REFUNDABLE

★ $375 TUITION IS DUE THE MONDAY OF EACH WEEK YOUR CHILD IS REGISTERED

★ CARD PAYMENT FEES: PAYMENTS MADE BY CARD WILL INCURE AN ADDITIONAL $13 PROCESSING FEE. CASH PAYMENTS ACCEPTED WITH NO ADDITIONAL FEE

I, the undersigned parent or legal guardian, hereby grant permission for my son/daughter named above to attend Camp On The Move - Adventure Awaits Recreation Program.

In the event that my child sustains an injury or illness while participating in the program, I authorize program staff and representatives to obtain and/or provide first aid and or seek necessary medical treatment for my child. I hereby release and hold harmless Camp On The Move - Adventure Awaits Recreation Program, its staff, sponsoring organizations, and representatives from any liability arising from the exercise of this authority.

I understand and acknowledge that participation in the recreation program involves inherent risks, including the possibility of physical illness or injury. Should such an incident occur, I authorize program staff and representatives to refer my child to a medical treatment facility (hospital, clinic, etc.) as deemed necessary. I further acknowledge that I am financially responsible for any medical expenses incurred as a result of such illness or injury.

By allowing my child to participate in the program, I acknowledge and accept these risks on behalf of my child and hereby release Camp On The Move Adventure Awaits Recreation Program and its representatives from any claims related to illness or injury sustained during participation. I also understand that my child is responsible for following all program rules and regulations and that failure to do so may result in consequences.

Toys, personal electronic devices and cell phones are not permitted during program hours. I also understand that the program is not responsible for lost, stolen, or damaged personal items, including but not limited to jewelry, electronics, cell phones, toys or other valuables brought to the program.

Additionally, I grant permission for my child to:

• Travel in vehicles provided by Camp On The Move - Adventure Awaits Recreation Program
• Participate in outings and field trips supervised by program staff
• Be photographed and/or video recorded while attending the program

I understand and agree that photos and videos may be used for promotional purposes, including social media and advertising.
