    By signing this form, I am pledging that I understand the Yellowstone Valley Animal Shelter (YVAS) offers public medical services only for those who are in financial need and/or those who would not be able to get their animals spayed or neutered through any other means. These appointments are NOT intended for people who are simply looking to save money, while those truly in financial need lose out. 

    CLINIC EXPECTATIONS & REFUND POLICY

    Refund requests must be submitted at least 48 hours before the scheduled appointment and owners will be charged a 25% processing fee.

    I agree to notify YVAS immediately if I am unable to bring my animal(s) for their scheduled appointment (noted above) so YVAS may fill my time slot with another animal in need.  Missed appointments will not be rescheduled or refunded.  No refunds will be given within 48 hours of the scheduled appointment. 

    I understand that my animal's procedure (from start of process to recovery post-op) may take several hours until discharged by clinic staff. I understand that while I am not required to stay on the immediate shelter premises I must stay in town and be able to quickly return if/when contacted via the phone number provided to YVAS. I understand all animals must be picked up before 5pm. Additionally, if the animal is then not picked up from the shelter within 24 hours they will be considered abandoned and become property of YVAS and may be put up for adoption. I agree to follow any/all instructions from shelter staff & volunteers. 

    RELEASE OF LIABILITY FOR MEDICAL TREATMENT

    I have the authority to grant the veterinarian my consent to receive, treat, and/or perform surgery upon the animal described on this form.  I understand that no assurance or guarantee has been made of the results of surgery or treatment and that risks and probabilities of complications exist in any surgical or medical treatment.  I understand that YVAS will use responsible precautions against injury, escape, or death of the pet; and I understand that some risks include (but are not limited to) anesthetic reactions, infection, opening of the incision, hemorrhage, exposure to potentially contagious illnesses, or death.  

    POST SURGERY ACKNOWLEDGEMENT

    I understand that I must keep my animal warm and sheltered after surgery and I also agree to follow all aftercare instructions. I understand that should I refuse to follow the recommendations of YVAS, YVAS is in no way responsible for the health and welfare of my animal. If you have concerns about your pet, within 72 hours of surgery, please contact YVAS medical at 406-294-7387 (from 9am to 5pm) to schedule a follow up appointment. The shelter may be able to provide your animal with additional aftercare. After 72 hours YVAS does not guarantee availability for follow-up care. You will be responsible for fees if prescription medication and/or additional surgery are required due to not following these post-operatative instructions. Please note that if you choose to seek treatment from your own veterinarian you are financially responsible for all charges. 

    In case of emergencies or after-hours care, we recommend that you take your animal to an emergency veterinary hospital; such as Moore Lane Veterinary Hospital, Best Friends Animal Hospital or Clarity Animal Hospital. Please note that if you choose to seek treatment from an emergency veterinarian you are financially responsible for all charges. 

    PHOTO & VIDEO RELEASE

    I understand that photographic or video recordings of my animals may be taken. By signing this release I am granting permission to YVAS to display these assets via social media or utilize for other marketing and educational materials on behalf of YVAS without payment or other consideration.
