Summer at St. Mary's Terms and Conditions
Medical Authorization
Parent hereby gives their consent, on their own behalf and on behalf of the Student, to have a School nurse, emergency medical technician, physician, nurse, dentist, or other healthcare professional, and in each case their associated personnel, provide the Student with medical assistance and/or treatment and agrees to be financially responsible for the cost of such assistance and treatment. Parent understands that treatment for any injury or medical emergency will be based, at least in part, on information provided herein and on the information provided on Student’s School medical form. Parent hereby authorizes emergency transportation of the Student, at the Parent’s expense, to a healthcare facility, should an individual listed above consider it to be warranted. If, in the opinion of a properly licensed and practicing physician, Student needs medical or surgical services which require Parent’s pre-authorization or consent, Parent hereby authorizes, appoints, and empowers the School to act as Parent and furnish such consent on Parent’s behalf. Parent confirms that it is their desire that Student be furnished with such medical or surgical services as soon as reasonably possible after the need arises. Parents hereby release and hold the School harmless from any liability which might arise from the giving of such consent. Parents agree to reimburse the School for any medical expenditures made on Student’s behalf.
Consent to On Site Medical Care
Parent hereby authorizes the School to supply medical care as needed for Student (including administration of allergy medications, Epi-Pens, inhaler, insulin, etc. according to the Student’s prescription from a licensed practitioner) and other minor medical care, as determined to be appropriate by the School nurse or other appropriately trained personnel. Parents hereby release and hold the School harmless from any liability which might arise from the provision of such medical care.
Sunscreen & Topical Application
Parent grants permission for camp staff to assist the Student with the application of sunscreen and/or insect repellent. Parents are responsible for providing these items if their child has specific skin sensitivities.
Behavioral Expectations & Dismissal
The School reserves the right to dismiss any camper whose conduct is deemed detrimental to the safety or well-being of the camp environment. This includes, but is not limited to, physical aggression, running away, or persistent disruption. No refunds will be issued for campers dismissed for behavioral reasons.
Late Pick-Up Policy
Camp ends promptly at 3:00 PM (or 6:00 PM for BTC Extended Day). Staff are not scheduled to supervise campers beyond these times. A late fee will be assessed for pickups occurring past the scheduled end time.
Student Photos, Voice and Images Release
Parent agrees to allow Student’s name, photograph, voice, video, image, and information (collectively “Information”) to be used by the School, and those acting with the School’s permission, for use in the School’s publications, promotional materials, website, and social media accounts, without compensation and without prior notice.
Parent also consents to the recording and distribution or live streaming of Student’s voice, image and video in instruction or presentations, as may be deemed appropriate in the School’s discretion.
Liability Release:
In consideration of the acceptance of my application for KaleidEscape, Parent hereby waives, releases, and discharges all claims for damages for personal injury or property damages which may hereafter occur to Student as a result of participation in this event. This release is intended to discharge in advance St. Mary’s Episcopal School and its employees and volunteers from liability even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing these risks, Parent hereby assumes those risks. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees.
Parent/Guardian Consent:
I give consent to my child to participate in the above activities, and I execute the above liability releases on their behalf.
