PARENTAL RELEASE FORM
I certify that my son/daughter is in good health and has my full permission to participate in the Long Island Junior Ducks Camp, which includes vigorous physical activity. He/She has no prior sickness, illness, disease, or bodily injury that would prevent safe participation.

I understand that physical injury may occur during camp activities. In the event that I cannot be reached, I authorize camp staff to obtain any medical treatment deemed necessary by a licensed physician.

I also understand that Long Island Junior Ducks Camp is not responsible for the loss or damage of personal belongings. I grant permission for the use of any photographs or video recordings taken during camp for promotional or publicity purposes.

REFUND POLICY
Long Island Junior Ducks Camp does not offer refunds if a camper is unable to attend after registration. However, the registration fee may be applied toward a future camp session. Refunds are not available due to fixed staffing costs and pre-purchased merchandise.

If a camp session is cancelled for the 2026 season, credit will be offered toward a 2026 camp session or toward another camp week during the 2027 season.

CANCELLATION POLICY
Long Island Junior Ducks Camp will follow all New York State, Governor, and local town regulations regarding camp operations. If a camp session cannot be held due to insufficient enrollment (fewer than 20 campers) or a New York State shutdown, families may transfer their registration to another available location or a newly offered week during the 2027 season.
