Liability Waiver*

I, the undersigned parent or legal guardian, acknowledge that my child’s participation in Kingdom First Sports activities involves inherent risks, including but not limited to bodily injury, illness, permanent disability, or death. I voluntarily assume full responsibility for all risks of injury or illness incurred by my child. I understand and agree that ALL medical care and expenses resulting from my child’s participation are my sole responsibility. Kingdom First Sports does not provide medical insurance coverage for participants. I hereby release, waive, and forever discharge Kingdom First Sports, its officers, directors, coaches, volunteers, staff, and affiliates from any and all claims, demands, or causes of action arising out of participation, including claims based on negligence, to the fullest extent permitted under Florida law. I agree to indemnify and hold harmless Kingdom First Sports from any claims or expenses arising from my child’s participation. I understand that my electronic signature is legally binding and equivalent to a handwritten signature.

If ever a situation arises where someone’s actions go against the principles our beliefs or the safety of the children, I agree that a peaceful solution may include asking that person to step away to protect the heart of the program. Kingdom First Sports’ greatest goal is to protect and care for every child—body, mind, and spirit—while honoring the traditions of our Faith Family. Everyone who works directly with children will undergo thorough background checks, ensuring a safe and loving environment.
