Supervision: Camp staff will accompany and supervise all children at all times.

  • I, the undersigned parent/guardian, give permission for my child to participate in supervised walking trips around the block to Stoffer Plaza as part of the Museum Summer Camp program. I understand that all reasonable safety precautions will be taken, and that camp staff will supervise the children throughout the outing.

On behalf of myself and my child, I voluntarily assume all such risks associated with participation in these activities.

I release, waive, and discharge the Museum, its directors, officers, employees, volunteers, and agents from any and all claims, demands, causes of action, or liability for any injury, illness, loss, or damage arising out of or related to my child’s participation in these offsite walking trips.
