ZĒL LIFESTYLE COLLECTIVE MEMBERSHIP AGREEMENT
This Membership Agreement is entered into by and between Tallmadge Chiropractic LLC, dba Zēl Lifestyle Collective, an Ohio limited liability company (“Zēl Lifestyle Collective”), and the undersigned Primary Member (“Member”).
By enrolling in the Zēl Lifestyle Collective Membership, Member agrees to the following terms and conditions.

MEMBERSHIP PRICING (MONTHLY)
• Primary Adult (first household member): $149/month
• Each Additional Adult (18+): $99/month

ONE-TIME ENROLLMENT FEE
• A one-time $350 enrollment fee applies to NEW patients only.
• The $350 enrollment fee is waived for EXISTING patients.

- Two chiropractic adjustments or dry needling sessions per month
- Three primary care visits per year
- Unlimited sick care visits
- Unlimited InBody scans
- Unlimited BrainTap sessions
- 20% off initial acupuncture visit
- 20% off microneedling
- 20% off all Zēl services
- 10% off labs
- 10% off supplements
- 30% off wellness packages
- One 60-minute massage at one-year anniversary (Primary Member only)
Independent Provider Services
Some services at Zēl Lifestyle Collective, including massage therapy and acupuncture, are provided by independent contractors. These services are not included in membership discounts, except for:

The initial acupuncture visit, which may receive a membership discount when used for the first time.
The complimentary one-hour massage offered to the Primary Member only at their one-year membership anniversary. This massage is paid for by Zēl Lifestyle Collective as a loyalty benefit and cannot be transferred or exchanged.
All additional massage therapy and acupuncture services are billed separately at standard rates.
LEGAL AGREEMENT

By enrolling in the Zēl Lifestyle Collective Membership, Member acknowledges and agrees to the following:

- Payment responsibility: Member is responsible for all membership fees and any applicable enrollment fees.
- No rollover / no refunds: Unused services do not roll over. Membership fees are non-refundable.
- Non-transferability: Membership is non-transferable.
- Primary member cancellation & continuation: Only the primary member may cancel the membership. Household members may continue if a new primary member is designated.
- Minor members turning 18: Child members who turn 18 must convert to adult membership.
- Cancellation policy: Membership may be canceled with written notice per the cancellation terms.
- Not insurance / no guarantees: Membership is not insurance. No guarantees are made regarding outcomes.
- Medical disclaimer: Services do not replace medical care. Seek appropriate medical attention as needed.
- Limitation of liability: Zēl Lifestyle Collective is not liable for incidental or consequential damages.
- Arbitration agreement (Ohio): All disputes will be resolved by binding arbitration in Ohio.
- Digital signatures: Digital signatures are valid and enforceable.
- Governing law (Ohio): This agreement is governed by Ohio law.
