Out of State - Illinois Consent Form

Use this form to submit your information and payment for State of Illinois fingerprint processing using an FBI-258 ink card.

This service is for individuals who will be fingerprinted outside of Illinois and need their prints processed by the Illinois State Police for licensing or background check purposes.

How the Process Works
1. Complete This Online Form and Upload Your ID

Enter your personal information (all fields marked * are required)
Upload a valid government-issued ID (Driver’s License, State ID, or Passport)
Submit payment online: Total Fee: $73.00
- $70.00 – Fingerprint processing
- $3.00 – Credit card processing fee

2. Print Out FBI-258 Ink Card and Get Fingerprinted Locally

Downloand and print an FBI-258 ink card (blue and white paper card)
Take the FBI-258 ink card to your local fingerprint vendor or law enforcement agency to get fingerprinted
Only one set of legible fingerprints is required
If prints are not legible, our office will contact you to request a second set.

3. Mail Your Fingerprints to TRACE2

Mail your completed FBI-258 ink card to our Chicago headquarters:

4. Processing & Confirmation

Fingerprints are processed within two (2) hours of receipt via FedEx
After processing, you will receive a completed OOS-FP form by email that includes your TCN number, which is used by the Illinois State Police and licensing agencies to track your fingerprints.

Important Notes:

Our mailing address shares a ZIP code with Elmwood Park, IL, but we are located in Chicago, IL
This service is per set of fingerprints and cannot be reused
Fingerprints and submitted files are securely destroyed within 45 days or sooner
No refunds and fingerprint cards will not be returned
