Get Fingerprinted and Mail Your FBI FD-258 Card

Step 1: Download and print the FBI FD-258 fingerprint card. Enter only your name. We will add the remaining information from your online form.
Step 2: Take the card to a local fingerprint provider or law enforcement agency. Make sure the fingerprints are clear.
Step 3: Mail your completed card ASAP to:

If we don't receive your ink card within 45 days, your application info will be deleted and you will have to complete and pay to submit your form again.
If you already have a completed fingerprint card, skip steps 1 and 2 and mail your card ASAP.

I am aware and understand that my fingerprints may and will be used to check the criminal history record information files of the Illinois State Police and/or the Federal Bureau of Investigation to include but not limited to civil, criminal and latent fingerprint databases. I also understand that if my photo was taken, my photo may be shared only for employment or licensing purposes. I further understand that I have the right to challenge an / information disseminated from these criminal justice agencies regarding me that may be inaccurate or incomplete pursuant to Title 28 Code of Federal Regulation 16.34 and Chapter 20 ILCS 2630/7 of the Criminal Identification Act.

How This Process Works

1. Complete the Form

Enter your personal information.
Upload a valid government-issued photo identification.
Pay online.

2. Get Fingerprinted
Skip this step if you already have a completed fingerprint card.

Download and print a FBI FD-258 fingerprint card.
Take the card to a local fingerprint provider or law enforcement agency for fingerprinting.

3. Mail Your Fingerprint Card ASAP

Send one clear set of fingerprints. We will contact you if another set is needed.

4. Processing and Confirmation

We process your card within two hours after receiving it.
We will email your completed OOS-FP form for Illinois or completed OOS form for Florida. The email will also include Transaction Control Number (TCN), TRACE2 credentials, and payment receipt.

• Our Chicago address shares a ZIP code with Elmwood Park, but we are location in Chicago

• Each payment covers one fingerprint submission.

• Fingerprints and uploaded files are destroyed within 45 days.

• Payments are nonrefundable, and fingerprint cards are not returned.
