CMSA Member Numbers are unique to each individual. You must submit your own Member Number for this event. In order to receive any discounts, your membership MUST be current and in good standing.

Platinum and Diamond Sponsors are invited to register up to ten (10) attendees at no cost for the Annual Conference.

Silver Sponsors are invited to register up to three (3) attendees at no cost for the Annual Conference.

If the company you list is NOT an annual sponsor, you will be billed for your registration. For a list of our annual sponsors, please visit our website: cmsa-easternok.org.

Conference Exhibitors are invited to register one (1) attendee at no cost for the Annual Conference.

If the company you list is NOT a registered Exhbitor, you will be billed for your registration.

Conference Registration includes welcome reception access for one person. Additional guests are $20 each. If necessary, you will be able at add additional attendees below OR on the next page.

CMSA of Eastern Oklahoma will be in touch to confirm your attendee count and collect payment.

If you are the guest of an Annual Sponsor or Conference Exhibitor, you do NOT need to select an attendee option. This is your opportunity to add additional attendees for the Welcome Reception OR lunch.

Attendees desiring CEU’s must complete this registration form (including their LICENSE NUMBER), sign in for each qualifying presentation, complete the accompanying speaker evaluation, and return their paperwork to CMSA of Eastern Oklahoma.
