I, the undersigned parent/guardian of this minor child, give permission for this child to participate in one or more of Akron Fossils and Science Center’s Summer Camps. I give permission for my child’s picture to be taken to be used on the museum’s website or in museum publications. I hereby give permission for the adult bearer of this document or a photocopy thereof to give consent for emergency medical treatment of my child by a licensed physician.

Please select your desired camps and add-ons. The full price is listed for your information, however only the $50 deposit is required in order to register your child. The full amount can be paid at any time before the final day of each selected camp, and MUST be paid no later than the final day of each camp.

If 'Morning Session' or 'Afternoon Session' is sold out, 'All Day' is also sold out. If a session you wanted is sold out, please select 'Waitlist' and you will be placed on our waitlist. No deposit is required to be placed on a waitlist.

Pricing
- Morning Session Only (9a.m. -12:30p.m.): $165 per week
- Afternoon Session Only (12:30pm-5pm): $165 per week
- All Day (9am-5pm): $240 per week
- Before Play Care (8am-9am): $35 per week
- After Play Care (5pm-6pm): $35 per week
- Before and After Play Care: $60 per week
When REGISTERING for ALL DAY, please SELECT BOTH morning and afternoon sessions. This applies when selecting both before and after care, as well.
