      We extend our heartfelt gratitude for your interest in participating in this significant ministry trip to Malawi, Africa, scheduled for August 2026. We are deeply blessed and immensely thankful to the Lord for the opportunity to share the life-transforming Gospel of Jesus Christ. As we carefully select the team members for this endeavor, we will earnestly seek the Lord's guidance and direction through prayer, ensuring that those chosen align with His divine will and purpose for this important mission.

      A mandatory, non-refundable deposit of $150.00 is required upon application submission. This deposit will reserve a place on the ministry trip, pending review and approval of your application. The application process will conclude on May 1st, 2026.If a deposit is not received with your application, your application will not be processed.       The total trip cost is USD $1,800.00 per person, based on double occupancy, which includes the $150.00 deposit. The full payment of $1,800.00 is required by June 1st, 2026. For married couples, the total cost is USD $3,600.00, with the same payment due date. Please ensure that both spouses submit individual applications, and include a note indicating that you will be traveling together. Please be sure to include "Malawi Team Payment" in the memo field of all payments.

     Payment Schedule:

    $150.00 - Deposit due with application

    $1,000.00 - Payment due by May 30, 2026

    $650.00 - Final payment required by June 1, 2026

                                                  

     The cost of the trip is all-inclusive and covers:

    * Quality hotel room with breakfast
    * Team meals (lunch & dinner)
    * Bottled water in your room
    * All in-country transportation (involving the ministry team)
    * Security during the crusade

     The deposit reserves your spot on the trip based on a completed application and acceptance. We reserve the right to deny any application that does not fit the conditions explained in this form.

                                             

     The following expenses are NOT covered:

    * International flights to Lilongwe
    * Entry Visa costs
    * Personal spending money
    * Snacks, coffee, ice cream, and other items outside of meal times
    * Gratuities
    * Immunizations
    * Traveler's or medical insurance
    * Extra expenses incurred due to medical necessity

     

     We want to keep the team together, so everyone will use the same travel agent for arrivals and departures. We'll send the travel details once you're approved. So, hold off on booking any flights until we get back to you and the trip is paid in full.

    We're basing the cost on having two people per room, so everyone will need a roommate. However, if we end up with an odd number of people, we'll try to make single rooms available, but they'll cost extra.

    In careful consideration of my acceptance by To Witness His Glory International Ministries as a designated member of the ministry team for the upcoming mission trip to Lilongwe, Malawi, meticulously scheduled from August 24th to August 31st, 2026, and with a full and complete understanding of the inherent risks and potential dangers associated with international travel, the specific ministry duties and responsibilities, and my active participation in this significant gospel crusade, I hereby, unequivocally and without reservation, voluntarily and knowingly agree to the following comprehensive terms, conditions, and a complete release of any and all liability. This agreement is entered into without any limitations, restrictions, or conditions whatsoever.

    I HEREBY DECLARE:

    I affirm that I am eighteen years of age or older. If I am under the age of eighteen, both the individual and their parent(s) or legal guardian(s) must initial and sign.

    I hereby affirm that I am in good health and, to the best of my knowledge, I am physically and mentally capable of participating in all activities associated with this mission trip. If applicable, I confirm that I have received or will receive, prior to the commencement of the trip, all vaccinations and preventative health measures as recommended by my local or state health department, as well as any specific health guidelines provided by To Witness His Glory International Ministries, for the countries and regions we will be visiting. I am fully aware that international travel, particularly within the context of a ministry or mission trip, inherently involves a spectrum of potential dangers and risks. These risks encompass, but are not limited to, hazards associated with various modes of transportation, including air travel, watercraft such as boats and rafts, ground transportation like jeeps, automobiles, buses, taxis, bicycles, and even travel on foot. The nature of the trip also exposes participants to the challenges of traveling in foreign countries, which may include traversing diverse terrains such as dense jungles, challenging mountainous regions, areas with high altitudes, and steep or unstable geographical features. There is also the possibility of interactions with individuals who may exhibit unfriendly behavior during meetings or other social gatherings. Furthermore, I acknowledge the potential for sickness or injuries in areas where access to adequate medical assistance might be limited, primitive, or even entirely unavailable, and where timely medical evacuation options may be constrained or non-existent. The trip could also expose participants to various unforeseen circumstances, such as exposure to crime, instances of civil unrest, the unpredictable forces of nature, or other unforeseen dangers. I fully comprehend that the previously mentioned and any other unforeseen possibilities constitute inherent risks linked to ministry and missions travel. I explicitly acknowledge that To Witness His Glory International Ministries assumes no responsibility for any injury, illness, or any other loss that I may experience. I understand that all medical expenses, personal expenses, and any other costs incurred as a direct result of any illness or injury sustained during this trip are my sole responsibility. I hereby unequivocally assume all risks associated with personal injury, potential sickness, or even the possibility of death, as well as any damage to or loss of my personal belongings. I also accept responsibility for any delays, alterations, or cancellations to travel arrangements, and any other damages or expenses I might incur due to my participation in this ministry/mission trip or any related activities. I agree to be fully accountable for my actions and decisions throughout the duration of this trip. Should I become ill, sustain an injury, or experience any other form of damage, I acknowledge that I am responsible for covering all related costs, including but not limited to, expenses associated with evacuation, and any medical care that I might require.

     I HEREBY VOLUNTARILY RELEASE, FOREVER DISCHARGE, AND AGREE TOHOLD HARMLESS AND INDEMNIFY To Witness His Glory International Ministries, ITS DIRECTORS,OFFICERS, AGENTS, EMPLOYEES, COORDINATORS, FACILITATORS,
    VOLUNTEERS, AND OTHER TEAM MEMBERS FROM ANY AND ALL LIABILITY,
    CLAIMS, DEMANDS, ACTIONS OR RIGHTS OF ACTIONS, WHICH ARE RELATED
    TO, ARISE OUT OF, OR ARE IN ANY WAY CONNECTED WITH MY PARTICIPATION
    IN THIS ACTIVITY, WHICH I NOW HAVE OR MAY HAVE IN THE FUTURE,
    SPECIFICALLY INCLUDING BUT NOT LIMITED TO THE NEGLIGENT ACTS OR
    OMISSIONS OF ANY PERSON SO RELEASED, HELD HARMLESS AND
    INDEMNIFIED, AND SPECIFICALLY INCLUDING CLAIMS RELATING TO ANY
    PERSONAL INJURY THAT I MAY SUFFER. 

     I agree not to file any claims, lawsuits, or make any demands for any injury, death, or loss that may arise from my participation in this activity.

     I agree to be responsible for any costs and/or legal expenses incurred by the trip leader(s), organizers, and/or participants as a result of any claim or lawsuit filed by me, or filed by any other party as a result of my conduct.

     I hereby consent to and agree to be responsible for the payment of any medical treatment rendered to me by any individual for any injury or medical condition arising during or as a result of my participation in this activity.  

     I hereby authorize To Witness His Glory International Ministries to arrange for my transportation and lodging for this trip. 

     I hereby agree that these promises, agreements, assumptions of risk, and releases are binding upon me, my family, all minors accompanying me or for whom I sign, and my heirs, legal representatives, and assigns. 

    I HEREBY MAKE EACH OF THE ABOVE STATEMENTS, ACKNOWLEDGEMENTS,
    AUTHORIZATIONS, RELEASES, DISCHARGES, HOLD HARMLESS AGREEMENTS,

                Biblical Discipline Actions We Will Follow If Needed:

     These procedures outline the steps our ministry leadership will follow in the event disciplinary action is necessary. To ensure clarity, please review the following procedures and sign the consent form. By consenting, you agree to receive correction, public rebuke, and/or removal if deemed necessary by our leadership. If issues of sin or disobedience arise, the following steps will be implemented to resolve the situation.

      *Scriptural Foundation:We base all actions on biblical principles of love, truth, and reconciliation (Matthew 18:15-17, Galatians 6:1).

      * Initial Approach: Gentle and loving confrontation with the individual, aiming for restoration and understanding.

     In the event of a disagreement with any individual, as followers of Jesus we should approach the matter with that person directly and with a spirit of gentleness, love and compassion of Christ Jesus. 

      * Repentance and Forgiveness: Encourage sincere repentance and offer forgiveness based on Christ's example.

     We as followers of Jesus Christ should bear with each other with a sincere repentant heart.Offering forgiveness,releasing bitterness,and showing compassion torward others. No matter the circumstance we are called to reflect the forgiving,loving attributes and character of God.If any one trespasses against us we are to forgive them, just as in Christ, God forgives us.(Colossians 3:13, Ephesians 4:32)

      * Confidentiality: Maintain confidentiality unless necessary for seeking counsel or protecting others.

     We are called to address an issue with descretion,love and respect. Aiming for  reconciliation,without involving any other members of the ministry team. Focusing on gaurding against gossip,protection of reputation and keeping unity. If the individual is of the opposite sex, please conduct this discussion in a location where others are present but cannot overhear your conversation.(Proverbs 11:13,Proverbs 17:9)

      * Restoration:Focus on restoring the individual and the broken relationships.

    We should walk in humility and examine our hearts and motives first rather than focusing on the other persons "fault".That way we can make sure our heart is aligned with God and His will for the situation.We should  immediately strive to foster trust and restoration in relationships. (Matthew 18:15,Matthew 7:3-5,Matthew 5:23-24)

      * Seeking Counsel: If the issue persists or is significant, seek guidance from mature, trusted Christian leaders.

    In the event that a resolution cannot be reached through direct communication, the individuals involved are required to present the matter to a ministry team leader for discussion. The ministry team leader will be expected to identify the root cause of the issue, determine any accountability, and facilitate reconciliation to the situation.If the team leader believes the problem persists, a subsequent meeting will take place, involving all pertinent parties, the team leader, and the trip coordinator, with the aim of achieving resolution. (Matthew 18:15-17)

      * Discipline as a Last Resort: If all else fails, consider church discipline as a final measure, following biblical guidelines.

     If the ministry team leader and event coordinator find any individual in rebellion to correction, our head ministers will be informed. They will bring definite closure to the situation, with all parties present to hear the final conclusion. Possible outcomes include:

    * The individual returning home within 24 hours.
    * Being brought before the whole ministry team for public correction.

    If absolutely necessary, the ministry team will be informed not to have any personal contact with the individual for the remainder of the trip. The individual will not be permitted to eat, sleep, or travel with any ministry team member.If an individual's conduct is deemed inappropriate and negatively impacts the team, the leadership team reserves the right to immediately send the individual home. (Matthew 18:15-17)

     

     To ensure a spiritually enriching and productive experience for all participants during our ministry trips, we firmly believe in upholding biblical principles and guidelines throughout our time together. These trips are specifically designed to foster spiritual renewal and revival, and we understand that such endeavors can, at times, encounter opposition. Therefore, we want to clearly communicate our expectations regarding conduct and behavior. With the scriptures as our foundation, we are committed to creating a safe, supportive, and healthy environment where everyone can minister effectively and experience the transformative power of the Gospel. We expect all team members to conduct themselves in a manner that reflects the character of Christ and promotes unity, respect, and spiritual growth within the group.

    We firmly believe that possessing robust spiritual connections with God and fostering supportive relationships among team members are essential for successfully accomplishing our mission objectives. Any actions that exhibit disobedience or involve sinful behavior can unfortunately create challenges and negatively impact the overall experience and success of the trip for everyone involved. Therefore, we are committed to providing loving and compassionate guidance and correction when such situations arise. We deeply value and greatly appreciate the cooperation and understanding of all team members regarding any necessary disciplinary measures that may be implemented to maintain a positive and productive environment for the entire group.

By signing, you grant full authorization to use any recordings or photos of you, in whole or in part, in any materials, media, and marketing channels, without limitations. You also agree that the ministry may edit the media. You agree not to use any authorized photos, videos, or media for personal or ministry-related promotion without written consent. To Witness His Glory International Ministries reserves all rights to its intellectual property, including audio/visual property, documentaries, photography, video recordings, and derivative works.
