    I hereby apply for membership of Lacrosse Western Australia.
    In consideration of my application for membership being accepted I acknowledge and agree that:

    IN THIS MEMBERSHIP DECLARATION:

    "Affiliated Club" means an affiliated member of Lacrosse Western Australia.
    "Claim" means and includes any action, suit, proceeding, claim, demand, damage, cost or expense however arising including but not limited to negligence but does not include a claim against a lacrosse organisation under any right expressly conferred by its constitution or regulation
    "Lacrosse Activities" means performing or participating in any capacity in any authorised or recognised lacrosse activity endorsed by Lacrosse Western Australia.
    "Lacrosse Organisation” means and includes LWA, LA and where the context so permits, their respective directors, officers, members, servants or agents
    “LWA” means Lacrosse Western Australia Incorporated
    "LA" means Lacrosse Australia Limited.

    IF MY APPLICATION FOR MEMBERSHIP IS ACCEPTED
    I will be a member of Lacrosse Western Australia. I acknowledge my membership will be deemed to be accepted upon my participation in lacrosse activities and I acknowledge that I will be bound by and agree to comply with the constitutions, regulations and policies of the lacrosse organisations of which I become a member.

    WARNING
    Lacrosse activities can be inherently dangerous. I acknowledge that I am exposed to certain risks during lacrosse activities including but not limited to physical exertion, contact with lacrosse equipment (including stick, ball, shoes, protective eye-guards and headgear), body contact, ground contact, playing conditions (including indoor and outdoor surfaces/venues), weather, game associated items and fixed periphery ground items. I acknowledge that accidents can and do happen which may result in me being injured or even killed, or my property being damaged.
    I have voluntarily read and understood this warning and accept and assume the inherent risks in participating in lacrosse activities.

    EXCLUSION OF IMPLIED TERMS
    I acknowledge that where I am a consumer of recreational services, as defined by any relevant law, certain terms and rights usually implied into a contract for the supply of goods or services may be excluded. I acknowledge that these implied terms and rights and any liability of the lacrosse organisations (or any of them) flowing from them, are expressly excluded to the extent possible by law, by this membership declaration. To the extent of any liability arising, the liability of the lacrosse organisation will, at the discretion of the relevant lacrosse organisation, be limited to the resupply of the services or goods or payment of the cost of having the services or goods supplied again.

    RELEASE AND INDEMNITY
    In consideration of Lacrosse Western Australia accepting my application for membership I:
    Release and will release the lacrosse organisations from all claims that I may have or may have had but for this release arising from or in connection with my membership and/or participation in any lacrosse activities; and
    Indemnify and will keep indemnified the lacrosse organisations to the extent permitted by law in respect of any claim by any person arising as a result of or in connection with my membership and/or participation in any lacrosse activities.

    FITNESS TO PARTICIPATE
    I declare that I am medically and physically fit and able to participate in any lacrosse activities. I will immediately notify Lacrosse Western Australia in writing through my affiliated club of any change to my medical condition, fitness or ability to participate. I understand and accept that the lacrosse organisations will continue to rely upon this declaration as evidence of my fitness and ability to participate.

    PRIVACY
    Lacrosse Western Australia collects, uses and discloses personal information for the purposes of conducting and administrating our organisation and other related activities, including for organising events, identifying participants, recording results and communicating with you, and for providing you with member services or promotional material such as other events or other sponsor offers. We collect, use and disclose personal information in accordance with our privacy policy. We may share your information with third parties as required, including but not limited to disclosing your personal information to other lacrosse clubs and partners. Our privacy policy contains information about how to access and correct personal information held by us, or how to make a complaint about the handling of personal information. Applications or services may be rejected if requested information is not provided. Opt-out procedures are available for individuals wishing to avoid receipt of promotional material from us or third parties.     SEVERANCE
    If any provision of this membership declaration is invalid or unenforceable in any jurisdiction, the phrase or clause is to be read down for the purpose of that jurisdiction, if possible, so as to be valid and enforceable. If the phrase or clause cannot be so read down it will be severed to the extent of the invalidity or unenforceability. Such severance does not affect the remaining provisions of this membership declaration or affect the validity or enforceability of it in any other jurisdiction.

    LA & NATIONAL POLICIES

    I acknowledge and agree that:

    1) I will be bound by and must comply with all Lacrosse Australia Policies, including but not limited to, the National Integrity Framework (Child Protection Policy , Member Protection Policy, Competition Manipulation & Sports Wagering policy, Improper use of Drugs & Medicine policy, and Complaints, Disputes & Discipline Policy), Anti-Doping Policy, and Lacrosse in Australia Code of Conduct and Lacrosse Social media policy, which can be found here https://www.lacrosse.com.au/nationalpolicies

    2) I acknowledge and agree that LA and SIA hold the right to update these policy as required and that the up-to-date policies will be published on Lacrosse Australia's website.

    3) I acknowledge that I am bound by these policies and it is my responsibility to be familiar with these policies;

    4) a breach of these policies could result in further action being taken, as outlined in the complaints or discipline procedures policies.

    These policies cover and apply to all players, coaches, officials, administrators, parents and spectators.

    I have provided the information as required on the registration form and overleaf/above and have signed this declaration and the registration form. I warrant that all information provided is true and correct. I acknowledge this membership declaration cannot be amended. If I do amend it my application will be null and void. It cannot be accepted by Lacrosse Western Australia.

    I have read, understood, acknowledge and agree to the above terms including the warning, exclusion of implied terms, release and indemnity.

    Where the applicant is under 18 years of age this declaration must also be signed by the applicants parent or legal guardian:

    I am the parent or guardian of the applicant. I authorise and consent to the applicant undertaking the lacrosse activities. In consideration of the applicant's membership in Lacrosse Western Australia being accepted, I expressly agree to be responsible for the applicants behaviour and agree to accept in my capacity as parent or guardian, the terms set out in this membership declaration, including the provision by me of a release and indemnity in the terms set out above. In addition, I agree to be bound by and to comply with the Lacrosse Western Australia constitution and any regulations and policies made under it.

    This Code of Conduct reflects our values and our commitment to community,
    sportsmanship, and integrity. It applies to every member of our club – players, coaches, officials, parents, supporters, volunteers and committee members. Together, we run with the pride.

    For Players

    • Play within the rules and spirit of the game.
    • Respect your teammates, opponents, officials and coaches.
    • Show good sportsmanship – win with humility and lose with grace.
    • Commit to training and match attendance and notify coaches early if unavailable.
    • Control your emotions as verbal or physical abuse has no place in our game.
    • Never use banned substances in accordance with Sport Integrity Australia’s rules.
    • Never play while injured against medical advice, and follow all injury and concussion protocols, including return-to-play procedures.
    • Represent Subiaco with pride as your behaviour reflects on the entire club.

    For Coaches and Team Managers

    • Promote participation, inclusion, skill development, and enjoyment above all.
    • Lead with respect, fairness, and clarity and be a role model for players and families.
    • Make decisions in the best interests of the team and individual wellbeing.
    • Enforce safety and injury protocols to prioritise long-term health.
    • Communicate openly with players and parents and listen with empathy.
    • Support club values and reinforce positive conduct in all interactions.
    • Maintain appropriate boundaries with all players.

    For Officials

    • Officiate fairly, consistently, and according to the rules.
    • Stay calm under pressure and be respectful in all interactions.
    • Wear the appropriate uniform and uphold the professionalism of the role.
    • Promote safety, sportsmanship and the enjoyment of the game.
    • Encourage rule changes that enhance inclusion and participation.
    • Maintain appropriate boundaries with all players.
    • Maintain appropriate and required training.

    For Parents and Supporters

    • Remember that children play sport for fun and focus on effort, not results.
    • Encourage and support all players, not just your own child.
    • Respect officials’ decisions and set a good example with your behaviour.
    • Do not use offensive language or behave aggressively toward anyone.
    • Applaud great play from both sides and reinforce the values of fair play.
    • In the case of injury, do not enter the field unless instructed by team staff or in an emergency.
    • Support our volunteers as they make participation possible.

    For Volunteers and Committee Members

    • Make decisions with transparency, fairness and in the best interest of the club.
    • Honour the trust placed in your role by listening, respecting and leading thoughtfully.
    • Share information clearly while respecting confidentiality where appropriate.
    • Promote inclusion and accessibility so everyone feels welcome and valued.
    • Use club resources responsibly and model pride in all club activities.

    Concerns and Accountability – Act with Respect
    We encourage open, respectful dialogue to raise concerns early.
    Please speak with a team manager, coach or committee member if an issue arises.
    Where behaviour does not align with this Code, the club may take action in line with our policies, which may include:

    • a warning;
    • temporary suspension from club activities; or
    • escalation through Lacrosse WA, Lacrosse Australia and/or Sport Integrity Australia

    Registration fees are now REQUIRED upon registration. Please select the number of games you expect to be playing. If you are unable to play the full season, you can be reimbursed.
