Please complete the form below and upload your photo. Please note that ONLY ONE photo per individual with Down syndrome can be submitted to the contest. You must check the box at the bottom of the page providing NDSS with consent to use your photo in order to be considered for the Times Square Video Presentation. Submissions will close on June 4, 2026 at 11:59 ET. You will be notified by July 8, 2026 if you are chosen.
