Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by HOCAS Group LLC during the selected summer classes. In exchange for the acceptance of said child’s candidacy by HOCAS Group LLC, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless HOCAS Group. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising from participating in, or returning from selected summer class sessions.

In case of injury to said child, I hereby waive all claims against HOCAS Group LLC. including all employees and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.

Medical Release and Authorization

As Parent and/or Guardian of the named participant I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to the HOCAS Group LLC. and its affiliates including Directors, Teachers, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Statement of Consent & Waiver

I hereby agree and declare that I am the legal parentguardian of the above-named child and hereby consent to the child's participation in the activities that are described to me in registration process. I understand that activities of the kind described may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities.

If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to activity supervisor(s) to make any decision and take any action to arrange for such procedures or treatments in the discretion of the activity supervisor(s).

I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the summer camp activities and I consent to the participation of the above-named child to these activities.

Any type of injury that may occur to the child without the fault of the camp management and activity supervisors, the camp management cannot be held responsible for any harm that may occur to the child without the fault of the camp management and activity supervisors.

  Program Enrollment Terms & Conditions
1. Tuition and Scheduling
• Standard Hours: Full-day sessions run from 9:00 am to 4:00 pm.                  Part-day sessions are available for Morning (9:00 am – 12:30 pm) or Afternoon (12:30 pm – 4:00 pm).
• Extended Care: Early drop-off (8:00 am) and late pick-up (5:00 pm) are available for an additional weekly fee of $50 per time block ($100 for both).
• Late Pick-up Fees: Children must be picked up promptly at their scheduled time. A late fee may be assessed for any pickups occurring after the agreed-upon window.
2. Payment and Refunds
• Accepted Methods: We accept Cash, Credit Card, Debit Card, and Venmo.
• Early Bird Discount: A 5% discount is applied to accounts paid in full by March 15th. Coupon Code: HOCASEARLYBIRD5
• Paid-in-Full Discount: A 10% discount is available for all tuition paid in full at the time of registration (online or in person). Coupon Code: PAYINFULL10
• Sibling Discount: Families enrolling multiple children are eligible for a sibling discount. (5% off each sibling enrolled) Coupon Code: HOCASSIBLING5 to be used after the first registration.
3. Attendance and Cancellations
• Tuition is based on the reserved space in our program, not on attendance. No refunds or credits will be issued for missed days due to illness, vacation, or personal scheduling conflicts.
4. Policy Agreement
By completing this registration and submitting payment, I acknowledge that I have read and agree to the pricing structure and terms outlined above. I understand that all fees are due according to the schedule selected.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

I hereby grant the releasee permission to use photographs in any of the following:

I hereby affirm that such release to the releasee does not constitute any form of compensation, including royalties arising from the photographs, to my benefit.

I understand and agree that photographs in the possession of the releasee shall become the property of the releasee. The use and publication of the photographs however, shall conform to my rights as a subject of said photographs.

I hereby waive my right to inspect of approve the photographs by which my likeness appears.

I hereby hold harmless, release, and forever discharge the HOCAS Group LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
