Camp fees are generously covered thanks to the support of the Native Village of Eyak and The Eyak Corporation for NVE tribal members and Eyak Corporation shareholders and descendants. No deposit is required. Eligibility will be confirmed with the NVE and The Eyak Corporation.

I Parent/Guardian * understand that my child's participation in camp involves inherent risks associated with recreation in the outdoors. Serious injuries can result from hiking over uneven terrain, being in and around water, and using tools for camp activities.

Minor injuries such as cuts, bruises, scrapes, and burns may occur. Risk of serious bodily injury is low but also present.

I assume these risks and agree to not hold Mavis Island Project and its employees and volunteers responsible if injury may occur at camp.

I certify that I have knowingly and voluntarily signed this agreement with the intent that it be a legally binding document designed to protect the Mavis Island Project , Prince William Sound Economic Development District, and Great Alaska Council from any and all claims which could be brought by myself or anyone else on account of injury or death to me or my camper regardless of cause or fault.

Consent for Medical Care

I agree to allow the Mavis Island camp staff and volunteers to administer basic first aid in the event that my child needs medical care. I understand that I will be contacted immediately if further medical care is needed. Staff and volunteers may transport my child to the emergency room at Cordova Community Medical Center (CCMC) if a major emergency may occur. I agree that CCMC may provide care in the event that I cannot be reached. I understand that I will be financially responsible for any costs related to care with or without insurance.

Payment

A $60 non-refundable deposit is due for each camp session. Your remaining balance invoice and any scholarship decision will be emailed to you within four days of submitting this form. The remaining camp balance is due two weeks before the start of each camp session. Online payments only. Cancellations must be made at least one week before the session begins. Cancellations made within one week of the start date are non-refundable. Mavis Island Project strives to ensure every child has the opportunity to attend camp.
