I (parent) understand that each camper is responsible for all medical bills incurred while at camp. Excess insurance coverage on each camper is provided by the String Music Basketball Camp, but only after the parent’s insurance coverage is exhausted. I also hereby give my parental consent to the camp physician in McComb, Miss., and at the Southwest Mississippi Medical Center, to evaluate and treat my child in any situation requiring medical attention. And I relieve all directors and staff of the String Music Basketball Camp of any and all liability resulting in acts dealing with my child during this camp

I also understand my son could be removed from camp for breach of camp conduct, specifically including, but not limited to, fighting, racial slurs, and being caught out after curfew. No refunds will be given in such a circumstance.

I allow the String Music Basketball Camp and/or Southwest Mississippi Community College to use photos of my son taken at camp to be posted to social media, or used in the future on the stringmusic.org website.
