  Please upload a photo/pdf of your Food License. If you are not licensed in Waterbury you should upload the license from your town and fill out the temporary food service application with Waterbury. We need that application returned and will file it with Waterbury prior to the event.
  
  Proof of insurance should also be uploaded naming CT Hurricanes (PO Box 214, Seymour, CT) as additional Insured.
  
  If you have a logo we can share to help promote the event please include it here as well.

- We are actively seeking sponsors to help defray the costs associated with this event. This is a great community event that we would like to run each year. Below is information on the sponsorship levels.
  
  Payment for the sponsorship can be made on the next page, or by mailing a check made out to CT Hurricanes to PO Box 214, Seymour, CT 06483. Please include a note indicating Pride and Class Sponsor and the sponsorship level.
  
  ● Bronze ($100): 2 Free Truck Entry, Name listed on the website & event banner
  ● Silver ($500): 4 Free Truck Entry or 1 Food TruckVendor Entry, Company logo on banner & website
  ● Gold ($750+): Company Fleet Truck Entry or 2 Food Truck/Vendor Entry, Company logo on banner & Website
  ● Platinum ponsor ($1500+): Gold benefits (unlimited truck/vendor entries) + Larger logo on banner + standalone banners at entrance + VIP recognition during performances
