CREDIT CARD AUTHORIZATION AND PAYMENT AGREEMENT

By registering for the Summer Baseball Camp, I expressly authorize Express Baseball Club, including its owners, employees, agents, affiliates, and designated third-party payment processors, to charge the credit card or debit card provided during registration for all amounts due under this Agreement.

I understand and agree that the total Summer Baseball Camp fee is $450 and will be collected in the following scheduled installments:

Initial Payment: $150 on {date}
Second Payment: $150 on July 1, 2026
Final Payment: $150 on August 1, 2026

I authorize Express Baseball Club to automatically process all scheduled and outstanding payments without additional notice or approval. I understand that declined, expired, blocked, disputed, or otherwise unsuccessful payments may be reprocessed multiple times and at later dates until all amounts owed under this Agreement are paid in full.

I acknowledge and agree that registration reserves a roster spot, staffing, scheduling, facility space, and operational resources immediately upon enrollment. Accordingly, all payments and fees under this Agreement are non-refundable and non-cancelable for any reason, including but not limited to injury, illness, scheduling conflicts, withdrawal, removal from camp, dissatisfaction, voluntary non-participation, or change of plans.

A “Late Payment” is defined as any payment not successfully processed on its scheduled due date. A “Returned Payment” includes any declined transaction, reversed transaction, disputed transaction, chargeback, or payment returned by the card issuer or financial institution for any reason.

I agree that:
(a) a $50 late fee may be assessed for any Late Payment;
(b) a 25% enforcement and collection fee may be assessed on any Returned Payment;
(c) I remain fully responsible for all unpaid balances, fees, collection costs, court costs, attorney’s fees, and administrative expenses incurred in enforcing this Agreement.

I understand that initiating a chargeback or payment dispute despite the existence of this signed authorization and participation agreement constitutes a breach of this Agreement. I agree that Express Baseball Club may submit this signed Agreement, registration records, payment history, communications, attendance records, IP address logs, and related documentation to the payment processor, card issuer, collection agencies, courts, or other third parties in response to any dispute or chargeback claim.

By signing below, I certify that I am the authorized cardholder or authorized user of the payment method provided, that I have read and understand this Agreement in full, and that I voluntarily authorize Express Baseball Club to process all payments described herein until all amounts owed are paid in full.
